2526174 Primary Facility 2016-10-06 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s James G. Beatty conventional gas well in Bell Twp. Clearfield County conducted October 6, 2016 at approximately 11 am. At the time of inspection; a service rig was rigged up on the well along with Universal cementers on location for plugging ops. When I showed up Universal was washing up the pump truck while the rig was pulling joints of tubing out of hole to clear the top of the balanced plug in the 4 ½” from bottom of the hole (3237’) back to 1248’ so Universal can circulate water on top of the plug to clean out tubing. I met with both the Cementer from Universal and Matt from Fairman Corp. to discuss operations on the pad. The Crew mixed 400lb bentonite w/water and pumped the gel ahead of balanced cement plug (3237-1248’) pumped in (3) stages to avoid getting workstring stuck in the hole due to the size of the plug . Cementer said the well went on a good vacuum after cement was displaced to balance and the tubing was pulling dry. Til I left the rig was done pulling all
2528667 Primary Facility 2016-10-14 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of Fairman Corporation’s James G. Beatty conventional gas well in Bell Twp. Clearfield County conducted October 14, 2016 at approximately 1:00 pm. The rig has left location following last week’s plugging operations. At the time of inspection; the well has 8 5/8” csg (w/threaded open csghead) a few foot above grade. The operator plans to weld 8 5/8” cap w/ 2” candycane vent monument. There was water inside the casing; no active bubbling or gas detected with Altair 5 x gas meter. No fallback; I tagged gravel a foot or two down . I dug up the annulus (11 ¾” x 8 5/8”); no gas detected. (Photo from today on Page 2)

Note to future inspectors: the lease road for this well is off Clover Run road next to the barn with metal red roof. The wooden bridge right at the entrance has been reinforced with metal strips. The operator intends to remove the strips so I would advised not to drive over the bridge, you will probably want to hike up to the well (about half mile) to look at it.

2785052 Primary Facility 2018-10-05 Site Restoration No Violations Noted Fairman Corporation has submitted a well site restoration report for plugged well 033-21356. Department representative Sue Herzing conducted a well site restoration inspection on October 5, 2018. The well has been plugged. The plugged well is represented by a labeled well monument with the well name and API number. The site has been restored and is well vegetated. The bridge leading to this restored well site has been removed by the operator.

Per this inspection the Department approves this well site restoration report.
