Plugging of this well was completed on 5/10/2017, a restoration report was received 5/31/2017 and approved 6/24/2017. This site and access road were both restored and stable with an adequate growth of vegetation. There has recently been an issue with the restored access road that prompted this follow-up inspection. The site and access road remain stable and vegetated however there are several ditches on the south side of the access road near the plugged well and associated pad area that need to be re-graded, seeded and mulched. There were also several pieces of pipe left near the ditches that need to be removed. This work should be completed during dry weather to prevent further issues. WQS Taylor discussed this issue with Alan Reken, EQT Representative, who agreed to complete the above mentioned work. |

Alan Reker with EQT was contacted about the issue and that the ditches would need to be backfilled and the area restored. WQS Taylor was notified by Alan Reker of EQT on 10/20/2017 that repairs were to be completed that day. This inspection confirmed that the ditches on the south side of the access road near the plugged well were backfilled and the area restored (seeded and heavily mulched) by an EQT contractor, Penn Mechanical, on 10/20/2017.
