2651503 Primary Facility 2017-10-24 Incident- Response to Accident or Event Violation(s) Noted On October 23, 2017 at 1545 Zach Subra of Range Resources called me to report a spill of produced fluids on Range Resources Gulf USA 40 Pad. At that time Mr. Subra was uncertain of the amount of produced fluids released but estimated at least 5 gallons. According to Mr. Subra, the well tender arrived onsite at 1415 on October 23, 2017 to conduct daily checks and/or services for the well and equipment on the Gulf USA 40 Pad. During the well tenders daily checks he discovered that a fitting on the 3/8" stainless steel supply line within the GPU (gas production unit) became loose, resulting in produced fluids being discharged every time the pump in the GPU kicked on. Produced fluids discharged inside the GPU and flowed outside of the GPU on to the surrounding ground. Mr. Subra had a roll-off box and excavator en route during our phone conversation. Mr. Subra was going to have a contractor start excavating the contaminated soil upon arrival at the pad.

2668981 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted The weather is 38 Deg. F and sunny, windy at time of inspection. Routine inspection of Range Resources Appalachia LLC, Gulf USA 40H Pad on December 6, 2017.

There is 1 producing well on this pad: (Permit: 37-027-21629). Both annulus (13-3/8¿ and 9-5/8¿) are open and vented at time of inspection. 0% combustible gas was detected at the annulus vents or in the area immediately surrounding the well. No violations noted. Range Resources Appalachia LLC representatives during inspection: Randy Hunter ¿ Environmental Compliance Specialist II, Casey Nicholson ¿ Lease Operator I. DEP Representative ¿ Ulyana Trynovich, Oil and Gas Inspector.

2678240 Primary Facility 2018-01-03 Follow-up Inspection Outstanding Violations - Viols Req'd Follow up inspection at Range Resources Gulf USA 40H well pad on January 3, 2018. No on was onsite during my inspection. There is one producing well (027-21629) located on this Marcellus well pad. In October 2017, an incident occurred with the gas production unit (GPU) connected to well 027-21629. Produced fluids were released from the GPU to the ground surrounding the GPU. During my inspection I observed soil excavated on the backside of the GPU. After leaving the site I contacted the Department Enviromental Clean-ups program. Tom McNerney is the lead inspector for the site remediation regarding the produced fluids release from October 2017. According to Mr. McNerney, Range Resources has recently submitted the Act 2 report to the Department for review. The Department is currently in the process of reviewing this report.

Violations sited on the October 24, 2017 remain outstanding. No new violations were observed during my inspection.

2811024 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Department representative Sue Herzing conducted a routine site inspection of Range Resources Gulf USA 40H well pad on December 6, 2018. There is one producing Marcellus well located on this well pad. The well head is properly labeled.

The ESCGP for this site expired in July 2015. This expired ESCGP did not call for any PCSMs when it was issued from the Department to the operator. Therefore, no PCSMs are in place. The site is stable. Slopes of the pad are stable with vegetation and rock. The site appears to be protected from accelerated erosion and sedimentation.

The Department recommends submitting a notice of termination for expired permit ESX10-027-0008 or applying for a renewal.

No violations noted.

2853625 Primary Facility 2019-03-13 Routine/Complete Inspection No Violations Noted The weather is 36 Deg. F, sunny and windy at time of inspection. Routine inspection of Range Resources Appalachia LLC., Gulf USA Pad, well # 40H on March 13, 2019. There is 1 active well on this pad.

This well is open and producing at the time of inspection. Annulus (5-1/5¿ X 9-5/8¿) is open and vented into the poly tank at time of inspection, 8% LEL and 0% combustible gas was detected at the annulus vent. Production processing equipment includes 1 gas processing unit, 1 production water tank and 1 poly tank, measurement equipment - orifice meter station. Production tubing pressure is 900 PSI, production casing pressure is 1100 PSI. Wellsite look neat and well organized. No concerns or violations noted at the time of inspection.
