2666166 Primary Facility 2017-11-30 Incident- Response to Accident or Event Violation(s) Noted I conducted an incident inspection at Alta Resources COP Tract 231 Pad D on November 30, 2017 with Tom McNerney of DEP and Rick Hall of Alta Resources. In addition, present on site were personnel from GES, MaHaffey Labs, and Tobias Machining & Welding.

There are a total of 6 drilled Marcellus wells on this pad, only one is producing (027-21589). According to Mr. Hall, Alta has a freshwater injection system set up for this well. A mixture containing biocide, corrosion inhibitor, and freshwater injects down the backside of the well casing. This fluids mixture is being pumped to the well casing via a pump located in secondary containment near the well heads. According to Mr. Hall, the steel piping was replaced earlier in 2017 with pex piping. The steel piping remains in place and the ends were cut and supposed to be capped. 2669283 Primary Facility 2017-12-08 Incident- Response to Accident or Event Outstanding Violations - Viols Req'd Rick Hall of ARD Operating LLC contacted me on December 5, 2017 at 1500 with an update regarding the release of freshwater mixture combined with biocide and corrosion inhibitor. The initial release reported to the Department on November 29, 2017 estimated 33-38 BBL of the freshwater mixture was released near the center of the well pad. Mr. Hall now reported a total of 132 BBL of the freshwater mixture is unaccountable for. According to Mr. Hall, further investigation of the freshwater mixture loss revealed another location where fluids were being lost. This location was found in the pex line between the produced fluids/freshwater containment and the gas production unit (GPU) containment (photo 1). The contractor revealed a gash in the pex line between both containment locations. The soil has been excavated, the pex line has been repaired, and the pex line has been back filled (photos 2-4). Mr. Hall stated that the operator planned on conducting more soil and water sampling.

Routine inspection of Alta Resources, LLC, C.O.P. Tract 231 Pad D Well # 1015H on December 15, 2017. There are 6 wells on location, 1 producing wells on this pad.

Well is producing and production gas goes to the main transmission line. Annulus is closed. Mechanical gauge reads 200 PSI on the production tubing at time of inspection. 50% LEL and 0% combustible gas was detected on ESD valve in the area immediately surrounding the well. Discussed gas leaking issue and follow up inspection with present company rep. on location. Well site looks neat and well organized. No violations noted. 2673825 Primary Facility 2017-12-21 Follow-up Inspection No Violations Noted The weather is 37 Deg. F and sunny at time of inspection.

Follow up inspection of Alta Resources, LLC, C.O.P. Tract 231 Pad D Well # 1015H on December 21, 2017. Previously 50% LEL and 0% combustible gas was detected at ESD valve. Equipment with gas leak was repaired and verified with a FLIR camera prior inspection. No violations noted.

2829219 Primary Facility 2019-01-17 Routine/Complete Inspection No Violations Noted Routine site inspection of COP Tract 231 Pad D on January 17, 2019. Well pads COP Tract 231 Pads C & D are both covered under the same ESCGP (ESX09-027-0003(01)). There are a total of six wells onsite; 1 is producing, 2 are in regulatory inactive status, and 3 are plugged. There are (4) 400 bbl produced fluids tanks located in secondary containment. Frozen water was observed within the produced fluids containment.

The well pad remains the working footprint size of 400' x 400'. PCSM have been installed. The site is snow covered and appears stable. No E&S issues observed.
