2524666 Primary Facility 2016-09-29 Plugging(Includes Plugged/Mined Through) No Violations Noted On September 29, 2016 I conducted a routine inspection of (3) plugged wells in Snow Shoe Twp. Centre County. The first well I found was the Gulf USA 102 (027-21260). The lease road is still navigable by vehicle and is (south) off of Kato Orviston rd about 1/10 mile East of caged pipeline leg manifold and corresponding ROW. At the time of inspection; Well is plugged, 13 3/8” conductor, 9 5/8” surface and 7” csg were all left in the hole. Annuli were open; no bubbling of gas detected with Altair 5x gas meter. The 7” was capped with metal plate with a 2” candy cane vent w/tag; no gas.

The next two plugged wells I found off of Black Bear road right next to the road, both permitted as API Permit # 027-21283 due to a plug and skid event (Stuck bit and DP @ 1170’, Shot off DP @ 635, P&A and Skid Rig). The well that had the bit stuck in the hole only had 13 3/8” conductor and 434’ of 9 5/8” surface casing. The 9 5/8” had a plate and 2” candycane vent; no gas detected, no bubbling in the 9 5/8 (x 13 3/8”) casing an
