    The well pad site was reduced to the post-construction footprint and restored to approximate original contour in 2016, and well site restoration reports for the (4) wells on the Whippoorwill location were approved by Department inspection in 2016. EQT submitted a notice of termination (NOT) for ESCGP ESX11-023-0001 in fall 2016, which includes not only the restored well pad, but also the access road upgrades, (4) stream crossings, and the freshwater impoundment site which adjoins the well pad to the immediate north.  After the NOT site inspection in late fall 2016, the NOT was not approved pending EQT performing re-seeding work on the southern portion of the impoundment LOD, where perennial vegetative cover failed to me

There are 4 producing Marcellus wells located on this partially restored well pad. According to Department records, this well pad was reduced in size during the spring 2016 to meet post construction standards permitted under ESX11-023-0001.

To date there are 4 wellheads, 4 modular separator/production units, and (4) 400 BBL produced fluids tanks which are stationed in modular secondary containment. I observed approximately 1" of frozen water within the produced fluids secondary containment. Four well site restoration reports have been submitted and approved for the current producing wells on this pad (023-20139, 023-20140, 023-20154, and 023-20157). The restored site is well vegetated. In addition, the ESCGP (ESX11-023-0001) that was issued for the construction of this pad has been previously terminated by the Department. No violations noted during my inspection.

2697779 Primary Facility 2018-02-21 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production. The wellhead assembly,all fittings,flow lines and vented annulus were tested for gas. Results of this testing was gas was detected on the 9 5/8" x 5 1/2" vented production annulus. No other gas was detected on the wellhead assembly or the well area.
