Based on current ES plan, department recommended that operator follow impoundment construction standards to construction pit on well site, since pit appears to be constructed in fill slope of pad, which were given to John Cenkner of Cenkner Engineering. Operator should ensure structural integrity of pit is maintained with adequate slopes, berm, and smooth compaction. Roadway for withdrawl of fluids may needed to be added. Compost sox and filter fence installed/will be. Based on review of this ES plan and discussed corrections, if implemented adequate controls and pit construction should be sustained.
1963252 Primary Facility 2011-03-30 Routine/Complete Inspection No Violations Noted Field meeting, requested by John Cenkner, conducted at proposed well site with Cenkner Engineering and Don Rose-contractor to review ES plan & discuss location/construction of pit(s) at the Rod and Gun # 2 staked storage well which is expected to be drilled in April/May 2011. Site currently being constructed. Discussed updates on ES plan, Cenkner Engineering followed department recommendations to follow impoundment construction standards for pit on well site, since pit will be constructed in predominately fill material as well as fill slope of pad. Since operator is constructing pit following impoundment standards (as recommended hence large pit is located in predominantly fill material, no cut slopes), department is ok with noted changes, department also recommended changes be discussed with driller to ensure encroachment on pad is ok. Bmp¿s installed and functioning as noted on ES plan. No accelerated erosion or sedimentation issues noted. Operator plans remove all waste (cuttings) offsite for disposal.
1967706 Primary Facility 2011-04-19 Routine/Complete Inspection No Violations Noted At the time of inspection, Site ID and ES plan present. Site gated. Well staked not yet drilled. Access road adequately rocked, no accelerated ES issues noted on access road. Well pad currently under construction. Since last inspection, below grade pit construction has begun: fill has been utilized to construction sides of pit, but base has not yet been dug as discussed in last site mtg. Pre-Spud meeting scheduled for April 20. All temporary BMP¿s installed and functioning according to plan, although it is recommended that filter fence be placed BEHIND brush/grub line; no accelerated ES issues noted on site. Although operator should continue to routinely monitor/maintain all BMP¿s until final restoration has been achieved at the site.

Sent copies of new regulations to Mark Shoemaker and UDI. Sent spud call information to Paul Sekinger. Followed up with concern of changing plan of well going deviated vertical to directly vertical relative to getting a new permit. Called Permit Chief and no new permit are required. Meeting moved to site for additional inspection. At the time of inspection, no E&S and IW issues. Site was in the process of designing well pit. E&S controls were in place. PNG was going to place the E&S plan at the site. Well sign and well stake present.
1981538 Primary Facility 2011-06-14 Routine/Complete Inspection Violation(s) Noted At the time of inspection, well ID sign and ES plan present. Well spud reported for 06/11/11. Rig onsite actively drilling, currently depth 1275ft. Observed one large pit onsite which appears properly lined, but diffuser placed on drill pipe appears to be splashing and/or releasing cuttings outside of pit (See Pictures);therefore, samples collected from ¿liquid¿ on the ground outside of pit, which appears to also be creating a possible ES concern. Spoke with driller rep onsite, and pointed out concern with pit and/or possible release of contaminated cuttings and ES concerns related to release. Bmp¿s were installed incorrectly, filter fence should be place behind brush grub line, and during pre-site construction meetings it was noted that compost sox would be utilized as a bmp as well due to well site being construction in HQ watershed. Accompanied by WQS Marc Maestra. Follow up inspection will be conducted, at this time inspection is pending final analysis results received from sample collection.
1985929 Primary Facility 2011-06-30 Drilling/Alteration No Violations Noted At the time of inspection, no E&S and IW issues. E&S controls were in place. Conductor pipe (13¿) ran to 60 ft. and grouted to surface. Halliburton cemented the 9 5/8¿ pipe into place used 266 sks Class A and 188 sks lite. 7¿ pipe set at 7698 ft with 560 sks of Tune lite and 125 sks of Class A with 15 bbls return. PNG used 60 centralizers at 125 ft apart.

1998795 Primary Facility 2011-08-25 Drilling/Alteration No Violations Noted At the time of inspection, no E&S and IW issues. Well completed. Pit remained with drill cuttings and fluid. 4 drums remained on pellets. Well head completed with manifold system. E&S controls present.
2002460 Primary Facility 2011-09-01 Routine/Complete Inspection Violation(s) Noted well sign E&S plan present at well site entrance. Access road adequately rocked and appears stable. Pad adequately rocked. Well drilled, not yet equipped for production. Upon examination of drill pit, it is identified that structural integrity of liner has been compromised due to holes in liner as well as separation of liner seam. During this inspection liquid fraction and drill cuttings were present in the pit. It is also noted that cuttings appeared to have been placed or accumulated on side of pit beyond the required 2ft of freeboard; therefore violations are being issued for operators failure to maintain pit liner to prevent permeability as well as failure to ensure 2ft of freeboard is sustained around ALL sides of pit. During inspection, left side of pit (when standing at well head facing pit) actively being pumped, but pit side mentioned above is not actively being pumped nor cuttings being removed. Bmps not maintained accelerated ES.

2010520 Primary Facility 2011-10-06 Compliance Evaluation Outstanding Violations - No Viols Req'd CEI inspection with Paul Sekinger, Ernie Moses from PNG as well WQS Supervisor Ed Ritzer, WQS Marc Maestra, and myself. At the time of inspection, pits have been removed and area re-graded, site has not yet been temporarily or permanently stabilized with vegetative cover and/or another acceptable permanent measure capable of resisting erosion. According to operator all pit contents were removed and hauled to a landfill and soil samples were taken of pit base prior to backfilling to ensure contents of pit/liner were not leached in to grounds of commonwealth. Bmp¿s installed down gradient and at the time of this inspection appear to be functioning, until final permanent restoration is achieved operator shall maintain all temporary bmp¿s.Operator agreed to stabilize previously disturbed areas within the next several days. Upon stabilization operator shall notify department and a follow up inspection will be conducted to evaluate site restoration. Operator plans to install a ditch and rock outlet to divert water below cut slope on well pad
2017453 Primary Facility 2011-11-02 Compliance Evaluation Outstanding Violations - No Viols Req'd At the time of inspection, well sign and ES plan present. Access road stable. PNG reps, Paul Sekinger and Ernie Moses present during inspection. Since last inspection, no evidence of temporary or permanent stabilization, as site has not been seeded/mulched. Site regraded. Bmp's are installed below gradient, although does not appear operator is routinely monitoring/maintaining bmp's as filter fence exhibited a buildup of sediment and compost sox stake has been removed in turn minor accelerated sedimentation noted outside bmp's. As of last inspection, operator planned to install a ditch and rock outlet to divert water below cut slope off well pad to direct natural runoff, but not yet completed at time of this inspection. Operator reiterated that bmp's would be cleaned and site temporarily stabilized within the next week ,as well as rock line ditch installed.
2018695 Primary Facility 2011-11-14 Compliance Evaluation No Violations Noted Since last inspection, site has been re-graded and temporarily stabilized with seed/mulch-although minimal growth observed, Bmp's are installed below gradient, and it appears that operator has removed some sediment located in front of filter fence and replaced stake in filter sox. Although operator should continue to closely monitor and maintain both bmp's hence, well site is not yet permanently restored to a state capable of resisting erosion. Also operator should continue to maintain bmp's as during this inspection it appears some sediment has accumulated between filter fence and compost sox. As of last inspection, operator planned to install a ditch and rock outlet to divert water(spring) below cut slope of well pad --it appears operator has added additional rock to toe of cut slope on back side of pad to help filter and direct groundwater (spring) which previously eroded areas of pad and slope small section of filter fence located below pad on slope has not been maintained or removed, nor has the piece laying on slope.
2022140 Primary Facility 2011-09-14 Follow-up Inspection Outstanding Violations - No Viols Req'd Meeting requested by the dept with PNG (Paul Sekinger) to discuss violations noted on 9-1-11 inspection¿partial follow inspection. DEP: myself, WQS Supv Ed Ritzer, and WQS Marc Maestra; PNG: Paul Sekinger(Consulting Engineer), Kurt Hundertmark(Manager Ops), Ernie Moses(Sr. Engineer), Joyce Dailey(Legal Counsel), and Missy Galovich(Well Tender). During this meeting, attention brought to ES controls below drill pit, as well as lack of seed and mulch along sides of fill slope of pit. Although recent heavy rains occurred, the dpt notified PNG on 9-2-11 of ES control failures, Dpt reminded operator that it is their responsibility to inspect BMPs until final restoration of site is achieved, especially after every rainfall event to ensure proper functioning ability of BMPs. Pit contents actively being removed. No coverved boxes onsite, if operator unable to clean pit before close of business today, operator shall cover pit with liner to ensure contaminatns due not leach onto ground. Samples shall be collected before pit back filled.
