1885798 Primary Facility 2010-05-19 Routine/Complete Inspection No Violations Noted According to records, well completed on 6/22/07 to a depth of 3,734 ft. Active permit since 3/15/07. WRAC submitted and approved on 7/16/07. Well head completed with 8¿ pipe reduced to 4¿ and reduced to 2¿ production line with 5 shut off valves. Production equipment consisted of separator (180 psi), data logger (150/0 psi), 100 bbl brine tank (<10 bbl fluid). No IW or gas leaks issues during inspection surrounding the site. Farm name on meter box. Slight E&S issues at the site. Site has needs additional restoration. Took pictures of the site. Request WQ personnel perform inspection of the site. Talked to Chad Wagner (Dorso) field supervisor about issues.
1890142 Primary Facility 2010-06-03 Routine/Complete Inspection No Violations Noted At the time of inspection well was drilled and in production. Access road showed signs of minor erosion, near entrance. Brine tank appeared sound. Restoration efforts were noted, but less than 70% successful vegetative growth was observed, as site appeared to be mostly rocked. No accelerated erosion or sedimentation issues were observed in relation to well site. Noted that O&G Inspector conducted Routine inspection on 5/19/10 (ID 1885798) and noted similar issues and spoke with Chad Wagner of Dorso, whom is aware of the concerns. A follow up inspection will be conducted to ensure maintenance activities have been completed as noted by this and previous inspection(s).
