2444808 Primary Facility 2016-01-15 Routine/Partial Inspection In Compliance with Policy According to Department records, well was spud on 2-24-84 and plugged on 10-15-85 with 8' of 2" vent pipe and a 2" vent top erected monument.

At time of inspection it appears access road has been removed and is restored. Site is snow covered. Department walked to coordinates located in Department records for this site and observed an open well casing; no erected monument located. Referring to OGI Paul Wyrwas for follow up inspection. Violations may be forthcoming pending results of OGI inspection. UPDATE 6-3-16: USPS returned inspection report to Department as "Not Deliverable as Addressed." Department recommends Operator update contact information to ensure timely delivery of inspection reports.
