Cellar for well was set while onsite, but not cemented yet. This should take place today. Hole for conductor has not been started yet so well has not been technically spud yet. There is a tank onsite where cuttings are being placed. Permit and Emergency Response signs are posted at the entrance to the pad. Along the access road there are sound walls that are 2 panels high. Entering the well pad, there are sound walls 4 panels high in the one corner closest to houses.
2640674 Primary Facility 2017-09-20 Routine/Partial Inspection No Violations Noted At the time of inspection, conductor for well has been set. 24" was set to 40'. This has been cemented into place. Well is now covered with a steel plate. Mouse hole was set beside the well.

Well has a cattle gate around it. Surface casing was cemented using 490 sxs Class A (106 bbls), 2% CaCl2, 2% Gypsum, 15.6 ppg, 1.22 yield. Displaced with 283 bbls and bumped plug. Floats held and 19 bbls returned to surface. A float shoe and float collar were used along with 5 centralizers. Intermediate casing was cemented with the same type, weight, and yield as above. 151 bbls were used. Displaced with 643 bbls water to land plug. Floats held and 32 bbls returned to surface. A float shoe and float collar were used with 15 centralizers.

Containment is down and chemicals are all within it. Drilling is being done using brine to the start of the curve, where it will be switched over to a synthetic based mud.

2687361 Primary Facility 2018-01-18 Routine/Partial Inspection No Violations Noted At the time of inspection, Nomac 79 was onsite drilling on the B 5H well. This well had production casing cemented using a lead of 320 sxs 35:65 Poz:Class H (63 bbls), 15.2 ppg, 1.1 yield and tail of 1375 sxs Class H Acid Soluble (383 bbls), 15.6 ppg, 1.56 yield. This was displaced with 315 bbls water. Plug was bumped and floats held. Top of cement was not reported. A float shoe and collar were used along with 222 centralizers.
2689477 Primary Facility 2018-01-30 Routine/Partial Inspection No Violations Noted At the time of inspection, no activity was taking place on the pad. Some drill pipe remained on the pipe racks. There were also a few roll offs and frac tanks on the pad. Containment has been removed and was sitting in the dumpsters. Site was covered with snow, but I did not see any indication of any environmental concerns. Well or pit were not covered and everything looked good.

Operator reported that flow back operations were underway at time of inspection and there were about 100 stages left in the stimulation.

I observed flow back water transfer operations for a period of time while on site.

Heavy rain event while on site.

2702271 Primary Facility 2018-03-05 Drilling/Alteration No Violations Noted At the time of inspection, C & J was onsite, but currently not pumping. Sound walls were extended over the weekend, extending the length of the northern side of the pad. They are 3 panels high. I can't judge how this affected the sound until they are back to stimulating. It was fairly quiet while I was onsite.

2703411 Primary Facility 2018-03-06 Routine/Partial Inspection No Violations Noted At the time of inspection, C & J was onsite stimulating on the 5H well. This well has had 12 stages completed. The 13th stage had been started, but coil and guns were lost in hole. They moved over to the 5H after this. They plan to fish this out after working on the 5H.

Decibel readings taken by OGIS were as follows: 75 dB inside of sound walls 65 dB just outside of sound walls 55 dB at the property line This is slightly less than before the additional sound wall was installed. Complaint ID is 332468

2704897 Primary Facility 2018-03-12 Routine/Partial Inspection No Violations Noted At the time of inspection, C & J was onsite setting up to perf stage 15 of well. This well has had 14 stages completed. Stimulating had been shut down n Saturday because operator had finished stage 26 on the 5H well and wanted to complete more on the 3H. A coil tubing rig was used to fish for tools that had been lost. They were retrieved on Sunday morning, then plug was drilled out.

Company man stated that more sound walls were going to be installed on the western side of the pad, near where access road meets the pad, then over to corner of pad where existing walls are in place. They expect them to be set up by tomorrow. Complaint ID is 332468

2705684 Primary Facility 2018-03-15 Routine/Partial Inspection No Violations Noted At the time of inspection, C & J was onsite stimulating well. New sound walls have been installed, connecting up to the sound walls recently installed last Saturday and extending along the edge of the pad to where the access road meets the pad. They are 3 panels high.

At the township building, the noise could still be heard, but it did not seem as loud as in prior days. I fid not perceive it as being very loud.

2709544 Primary Facility 2018-03-19 Routine/Partial Inspection No Violations Noted At the time of inspection, C & J was onsite, but no stimulation was occurring. This well has had 44 of the 47 stages completed. Company Production Superintendent said that they hope to be done with all stimulation stages by Thursday or Friday.

2742624 Primary Facility 2018-06-14 Routine/Partial Inspection No Violations Noted At the time of inspection, well is still being worked on to be brought into production. Well has a Christmas tree installed on it, but the flow line has yet to be hooked up. There were some workers on the pad doing this, but it didn't appear that it would be completed for a few more days. There has be no earth moving activities yet. Cellar has been filled in to surface with gravel. 4 tanks and 3 line heaters have been placed within lined, steel dikes. The pipeline work is continuing off in the distance from the pad.
2760328 Primary Facility 2018-08-02 Routine/Partial Inspection No Violations Noted At the time of inspection, well remains shut in until pipeline is completed to the pad. Company representative stated that this could take a few more weeks. All of the piping needed to produce on the pad has been completed flow lines, emergency shut off valves). More stone had been added and reworked. 4 tanks are set within a lined, steel dike and 3 line heaters within another dike. A gas buster has also been installed.

No activity was taking place during inspection. There were 2 frac tanks that remained on the pad.

2803547 Primary Facility 2018-11-14 Routine/Partial Inspection No Violations Noted On site for a routine partial inspection.

Well drilled, completed, and in production. Well tag in place. Well protected by fencing and jersey barriers. Production equipment set (production tanks and GPUs) in secondary containment. Combustor installed. Gathering pipeline installed.

Site stabilized, PCSM BMPs installed and 100% vegetated.
