2664007 Primary Facility 2017-11-16 Routine/Partial Inspection No Violations Noted At the time of inspection, a Soilmec SR 30 auger rig is onsite drilling mouse holes. Well was spud and has 24" conductor set to 42'. Cellar is also set. Well was covered with a steel plate. Cuttings were being sent to a roll off. There were sound walls set up on the eastern and northern sides of the pad. Location is stoned. The Emergency Response sign and permit sign are posted at the entrance to the lease access road.
2671857 Primary Facility 2017-12-07 Routine/Partial Inspection No Violations Noted At the time of inspection, Appalachian Drillers rig 1 was onsite drilling on the 2H well. This well had surface casing cemented using 530 sxs Class A (115 bbls), 2% Gypsum, 2% CaCl2, 15.6 ppg, 1.22 yield. Displaced with 85 bbls water and landed plug at 210 psi. Pressured up to 760 psi and floats held. 7 bbls cement to surface, which fell back down about 9 feet. This was topped off. A float collar and float shoe were used with 5 centralizers. Intermediate casing was cemented with 700 sxs (152 bbls) as same cement used on surface job. Displaced with 155 bbls processed water and landed plug. Floats held and 30 bbls returned to surface. Float shoe and float collar used with 16 centralizers.

The 1H had production casing cemented using a lead of 340 sxs 35:65 Poz Class H (67 bbls), 15.2 ppg, 1.1 yield and tail of 1240 sxs Class H, 35% Acid Soluble (346 bbls ), 15.6 ppg, 1.56 yield. This was displaced with 305.5 bbls water with inhibitor. Plug was bumped and floats held. 6 bbls of spacer returned so calculated top of cement is at 5100'.

Major modification to address infiltration basin submitted by the operator to the Department. Vegetative growth established in previously disturbed areas.

Site consists of two wells in production the 1H-019-22645 and 2H -019-22646). Production equipment consists of three 400 bbl tanks, 2 GPus and TVO. A Well Site Restoration Extension request was submitted by the operator on 8/31/2018 for the purpose of addressing stormwater issues. A major modification has been submitted and extension will be granted until 8/30/2019.
