2816659 Primary Facility 2018-12-17 Routine/Partial Inspection No Violations Noted On 11/20/2018, WQSS Scott Lux and WQS Brian MacQuarrie onsite for the purpose of conducting a Notice of Termination inspection for ESCGP ESX14-019-0054. The well site consists of five wells in production; 1H (019-22267), 2H (019-22417), 3H (019-22418), 4H (019-22419) and 5H (019-22420). Production equipment onsite includes two 500 bbl tanks installed within secondary containment structure and five GPUs. Post Construction Stormwater Management Dry Extended Detention Basins installed and maintained. Permanent stabilization , under 25 Pa. Code 102.22(a)(2) has been achieved.

On 12/14/2018, an amended Notice of Termination application was received from the operator. The new application included Appendix A,B and C which was absent from the original submission.

Therefore, the Notice of termination request is approved.
