Well site has one drilled well. Well is on inactive status. Well head in place. No production equipment installed. No recent activity noted. Cattle guard installed around well head. Site stabilized with vegetation growth. All installed BMPs functioning properly.

Act 13 sign installed.

No well tag in place.

The Department requests that a well tag with permit number be installed.

2726775 Primary Facility 2018-05-04 Plugging(Includes Plugged/Mined Through) No Violations Noted BJ cement equipment onsite. Cudd Energy Services crane equipment on site. Coil tubing rig on way to site and expect to be rigged up and pumping cement at 2PM.

Plan to tag bridge plug and spot cement 200' above plug using 391 sks of cement.

2727662 Primary Facility 2018-05-07 Plugging(Includes Plugged/Mined Through) No Violations Noted At the time of inspection, no activity was taking place at the site. Containment remains on the pad and secondary containment (with berms) surrounded the wellhead. The top of water can be seen in the well through what remains of the wellhead. It would be near ground level. Some of the cement covered the well from overflow and pea gravel was noticed in the secondary containment. A BJ Services cement truck is still onsite. 2 frac tanks and a open top tank are still on containment. The open top tank only has a few inches of freeboard remaining, if this has any contaminants in it, it could be considered a spill if it overflows. Please take care of this ASAP.
2737088 Primary Facility 2018-06-04 Administrative/File Review No Violations Noted
2809296 Primary Facility 2018-11-28 Site Restoration No Violations Noted On site for a site restoration inspection.

Operator submitted a Post-Plugging Well Site Restoration Report.

Well is plugged with plugging marker buried. All BMPs have been restored. Site has been restored to approximate original contours. Noted sufficient vegetation growth. Snow coverage at time of inspection.
