2699763 Primary Facility 2018-02-27 Routine/Partial Inspection No Violations Noted On 02/27/2018, WQS Brian MacQuarrie onsite. Site sign maintained at entrance. Ten wells installed and tagged. Nine in production. The 1H Permit No. 37-019-21878 currently in inactive status. Tank battery consists of eleven 400 bbl tanks installed within secondary steel containment structure. Freshwater impoundment installed and fenced in adjacent to well pad. No apparent issues.
2715710 Primary Facility 2018-04-04 Routine/Partial Inspection No Violations Noted At the time of inspection, well was on inactive status, so there was no reading on the tubing gauge. Well was properly labeled and the Emergency Response sign is posted at the lease entrance. There was a very large lined, steel dike that had 7 tanks on one side and 4 on the other. All except 1 are for produced water. Each tank is labeled for which well it is associated with. The separators are also labeled. There are 10 of these with line heaters and are between the tanks. There was also a lined impoundment along the lease road. It still had plenty of freeboard, even after the heavy rains.
2819691 Primary Facility 2018-12-27 Administrative/File Review No Violations Noted Operator submitted the required notification under 78.103 to give the Department three working days prior notice of the annual monitoring and mechanical integrity testing. Operator is scheduled to perform this test on December 19, 2018. Notification received by email on December 13, 2018.

Ten gas processing units installed within containment structure. Freshwater impoundment fenced in and locked. Well sign and access road maintained. Previously disturbed areas vegetated and restored. Erosion and Sedimentation BMPs maintained.
