2700528 Primary Facility 2018-03-01 Routine/Partial Inspection No Violations Noted At the time of inspection, operator was concerned that his equipment could have been damaged or stolen, so I was asked to inspect. The property owner refuses access for operator on to the property. The location has equipment being stored on it. The rods that were onsite have been moved aside for equipment to move around. There is still a plastic tote at the site. Several pictures were taken for operator to see what the area looks like and to see if any equipment is gone. Operator knows he is under order to plug this well.
2800804 Primary Facility 2018-11-07 Compliance Evaluation No Violations Noted At the time of inspection, the well was not producing. It does not look as though the well has been touched since my last inspection. PennDOT had done some bridge work and silt socks were laying adjacent to the well site.

The rods and a plastic tote from the previous inspection were still near the well and the pump jack was still onsite. The well integrity appeared to be intact, with no signs of any leaks.

2823388 Primary Facility 2019-01-02 Follow-up Inspection Violation(s) Noted Records show violation issued for abandonment on 8/18/15. Abandoned Well was part of an Order dated 9/26/16. COA was issued on 5/18/17 and supersedes the Order. COA required the well to be plugged by 12/31/18. Inspection shows well remains abandoned with pump jack and flow line disconnected from the well (picture attached). Well is in non compliance with with the COA.
