2735836 | Primary Facility | 2018-05-30 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, Toby Dishong had a cable tool rig onsite. This well will be cleaned out and, depending if gas is encountered, will be sold to produce or plugged. There was no activity taking place during my inspection. Speaking with Dishong on phone, he stated that they have been attempting to pull the 4 7/8" casing out and then run his own 4 1/2 casing with packer into the well to shut off the freshwater. So far, the well has been shot at 1000', 960', and 900', but the pipe wont come free. They plan to bring in a different company to try shooting again. They tagged the top bridge plug at 1500'. There was an open top tank, 50 bbl plastic tank, and several plastic totes at the location. There were also 2 pipe trailers, one of which had 4 1/2" on it and the other was empty and sitting at the well. Some tubing on pipe racks was setting along the access road for use later on. |
2741189 | Primary Facility | 2018-06-07 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, Surefire wireline was onsite perfing holes in 4 7/8" casing. In the event this well has to be plugged, casing was perforated at 1400', 1300', 1200', 1100', and 1050' for cement to squeeze. Several attempts were made to pull 4 7/8" in hopes to replace with 4 1/2", but casing wouldn't budge. Surefire was able to make a clean separation at 810' and 4 7/8" was being pulled and torch cut from there. Dishong said he was going to pull out 150' of casing then have Surefire locate the 7" packer. |
2747486 | Primary Facility | 2018-06-22 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, I inspected some samples taken from plugging operator from around 1540' and down to 1734'. They appeared either shaley at the upper depth to a thick gel material which was very sticky. Also, some wood and other garbage has been encountered. The integrity of the hole was good and no caves. With no records during the inspection, we believed it was plugged back. The operator said the well has been shut in since he started working for his Mom (original operator). That was 40 + years ago. Well was logged and I decided to approve attainable bottom.
Later in day I received a call that a plugging record for well was found from 1969, in which well was plugged back to 1485' with fire clay and drill cuttings. Plugging operator was also able to get tools to 1750', which will be the attainable bottom now. |
2747504 | Primary Facility | 2018-06-26 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, Toby Dishong was onsite getting well ready to cement. The location has been too wet to get cement trucks to site so no work has really been done with well since last inspection. 1 1/2" tubing was put on bottom and well has been baled for water. TD still remains at 1750'. |

2754407 | Primary Facility | 2018-07-18 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, no activity was taking place. So far, cement has been poured from TD (1750') up to calculated 830'. The cement top was tagged the following day at 865'. Another stage will be cemented when a cement truck is available. A pit was dug which has some fluid in it. This may only be rain water. There was also a hose laying next to the well which led to an open top tank. This had some fluid and residual waste in it. |
2756067 | Primary Facility | 2018-07-24 | Routine/Partial Inspection | No Violations Noted | At the time of inspection, no activity was taking place. Another cement job took place last Friday. A solid cement plug was set from 865' up to a calculated 300'. This was tagged the following day at 272'. Well was then filled with pea gravel to surface.
A marker has been made up which has the API # welded on a plate, but has not been set in the well yet. The rig has been moved up the access road and the pipe trailer was taken offsite. Some reclamation needs to be completed yet, but the well itself is plugged. |
2760334 | Primary Facility | 2018-08-02 | Plugging(Includes Plugged/Mined Through) | No Violations Noted | At the time of inspection, the marker has been set below ground level and filled in. There was still an excavator and open top tank onsite. The tank had been emptied and contaminants disposed of and then it was flipped over to prevent rain water from filling. |
