The department may conduct follow-up inspections of well sites and related activities to determine compliance with this chapter. All E&Scontrols in place. Site built to plan design. No violations noted.

Site preoperational approved on 6/5/2018.

2752967 Primary Facility 2018-07-18 Routine/Complete Inspection No Violations Noted Weather was 71° and sunny at time of inspection. There were no Chief representatives on location at time of inspection. Well is a spud conductor. A steel plate was welded over the top of the conductor casing at time of inspection. The mouse hole was also set and open. No violations noted at time of inspection.
