(a.1) Preoperation inspections.--The operator may not commence drilling activities until the department has conducted an inspection of the unconventional well site after the installation of erosion and sediment control measures. The department may conduct follow-up inspections of well sites and related activities to determine compliance with this chapter. All E&Scontrols in place. Site built to plan design. No violations noted.

2701615 Primary Facility 2018-03-06 Drilling/Alteration No Violations Noted Two well cellars are being built by a development crew. All removed soil and gravel being placed within roll off containers. No other equipment on location.
2812025 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Two wells have recently been completed and are in flowback. Two frac tank batteries are on location within secondary containment. Most the pad surface and the surface within the secondary containments was ice-covered.

Two steel produced water tanks are present and in good condition. These sit within steel containment with spay-in liner in good condition. There was no water within this containment. Two GPUs are present within containment. The water within the containment was unfrozen and had a conductivity of less than 100 uS/cm.

Two sediment traps appear to be built as permitted and functioning as designed. Compost filter sock is deployed as the E&S plan calls for. Site is stable. No E&S issues noted.
