The department may conduct follow-up inspections of well sites and related activities to determine compliance with this chapter. All E&Scontrols in place. Site built to plan design. No violations noted. Site approved for drilling activities.

2687396 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted I met Trey Villareal of Chief when I arrived onsite. H&P rig 513 on location at 2970' on the 3H well. Crews were cleaning ice and snow out of containment. I noted a section of berm near the mud logging trailer where the lock and key style berm was not run straight, but made a s-curve instead; in this area, there were several spots where the berm sections did not meet fully, and the liner was sagging between them. Near the cuttings backyard, I noted several spots where the liner on the berm was torn, likely from vehicular traffic in the area. I then met Aaron Stredny, Environmental Specialist for Chief, and pointed out the containment issues I had noted to him. Aaron took pictures of the aforementioned areas, and we spoke for a few minutes regarding solutions for this and other issues that we had noted on other locations. I informed Trey of my findings as well before I departed.

No rig matting on location. Only polyurethane and felt liner.

No leaks detected. A small hole in the secondary containment was noted, but was marked by the operator for repair.

2762531 Primary Facility 2018-08-08 Incident- Response to Accident or Event No Violations Noted I spoke with Chief's Environmental Inspector, Aaron Stredny, who is guiding the cleanup of flowback that sprayed out of a gas buster trailer (photo 1). He said they started flowing back the six Marcellus Shale wells last Saturday (4 days ago), and that the gasbuster overflowed last night.

There was just enough liquid in the excavated area (photo 1) for me to get an electrical conductivity (EC) reading with an Oakton PCSTestr 35; it was 1.1 mS/cm. From puddles on the rest of the pad, the highest EC reading I got was 0.50 mS/cm. 78.56a is cited to the 015-23364 permit. The violation is considered immediately corrected. JLP

2813381 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Two frac tank batteries are on location within secondary containment. No problems with the containments were noted. Unlabeled containers are present near the well heads. The department recommends labeling these containers.

Six steel produced water tanks are present and in good condition. These sit within steel containment with spay-in liner in good condition. The water was frozen within this containment. Six GPUs are present within containment. The water within the containment was unfrozen and had a conductivity of less than 100 uS/cm.

Two sediment traps appear to be built as permitted and functioning as designed. Compost filter sock is deployed as the E&S plan calls for. Site is stable. No E&S issues noted.
