2657054 Primary Facility 2017-11-01 Routine/Complete Inspection Pending Cold/raining at time of inspection. Cellar was full of water when i arrived on location. Jon haffer with chief oil and gas accompanied me during the inspection. There were vac trucks on location used to suck the water from the cellar, once the water level was lowered in the cellar there was bubbling observed outside the conductor pipe. Jon haffer was present at the time and had observed the bubbling as well. It should be noted that jon and myself discussed the bubbling and agreed that the bubbling will be monitored on a weekly basis, and that jon will get a Methane/LEL reading from the cellar as soon as he has the chance. I was unable to get. A picture at time of inspection due to rain.

2730028 Primary Facility 2018-05-15 Routine/Complete Inspection No Violations Noted Arrived at 11:30. Weather: windy but nice. There were one shut in well and three conductor only spuds on location. There were six empty oil based mud tanks. Rig mats and liner were still down. No violations were observed at the time of this inspection.
2739663 Primary Facility 2018-06-11 Routine/Complete Inspection No Violations Noted On 6/10/18 a drilling resumption after break of 30 days or more notification was submitted to the Department. The date and time of this action will occur on 6/12/2018 at 10 pm.

The Department arrived on 6/11/2018 for a inspection of containment and E&S controls. All conditions on the well pad were acceptable. The setup of Precision Rig 569 and equipment is underway. No violations noted.

2770346 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted The weather was 71° and partly sunny at time inspection. There were no Chief representatives on location at time of inspection. Well had a 0-5k psi gauge attached above the swab valve on of the the wing valves off the well head. The gauge off the swab valve appeared to be closed in and read 0 psi. The gauge on the wing valve was open and read approximately 1550 psi at time of inspection. There was also a digital gauge on the same wing valve that read 1555 psi at time of inspection. This well also had a pay line plumbed to it above the swab valve with a 0-5k psi gauge attached to it. It was closed in and read 0 psi at time of inspection. There were 2 annulus vents plumbed to surface at time of inspection. One the the vents was the 9x5 annulus vent and the other was the 13x9 annulus vent. Both vents were closed in with bull plugs at the well then plumbed together into a poly vent tank at time of inspection. Vents were open to the poly vent at time of inspection. 2850915 Primary Facility 2019-03-08 Routine/Complete Inspection No Violations Noted Inspection conducted in response to a request for a well site restoration extension and to assess overall site conditions. Four wells are staged for production. Two wells are appropriately labeled with their respective well permit numbers and two are not. Two unlabeled white plastic containers are staged near the well heads. The Department recommends labeling the containers. A battery of frac tanks is set-up within secondary containment. The tanks and containment appear to be in good condition. Four roll-off boxes, a tractor trailer, and a vac tank were also present on the pad. One steel produced water tank is present within steel containment with spray-in lining. There is minor corrosion on the produced water tank and the thief hatch on the tank appears to be open. The containment is in good condition. Four GPUs are present within secondary containment. The GPUs and containment are in good condition overall.
