2667868 Primary Facility 2017-11-30 Routine/Complete Inspection No Violations Noted I arrived at the site at 11:43. Weather conditions were mostly sunny and breezy with temperatures in low to mid 40's. I met with Larry Patrick and Bernie Saxe well testers of Oil States International Incorporated, a subcontractor of Chief Oil and Gas conducting flowback operations. Mr. Patrick and Mr. Saxe stated the following in reference to site activities. "Flowback is in progress. The 1H-3H wells are currently being flowed back. We started flowing back the wells last night at 5 PM. We are just getting water from the wells now. No natural gas yet. Natural gas will be sent to sales once we get it. The smaller blue tanks mounted on the truck trailers are vessels/separators that can separate oil, gas, water and solids/sand. The large blue tanks mounted on the trailers are line heaters. Fluid goes into line heaters first and then to separators. The blue upright tanks near the wells are sand traps that collect sand as the wells are flowed back. Fluid and sand in the traps is transferred into a frac tank near t
2717970 Primary Facility 2018-03-29 Routine/Complete Inspection No Violations Noted I arrived at the site at 12:49. The weather conditions are cloudy skies with a light breeze, occasional wind gusts and temperatures in the low to mid 50's. There was no activity or personnel at the site. The Ambrosius Unit B 1H-3H wells are in the production phase of operations. I observed trash scattered on the ground surface at the site. Trash was observed on the ground surface at the site during an inspection conducted by the Department on 11/30/17, inspection ID # 2667886. I performed field screening for specific conductance (conductivity) in water on the pad surface, site perimeter stormwater ditch on the fill slope of the site, sediment trap on the fill slope, and production fluid tanks and natural gas production units containment structures. The conductivity was elevated in water in the production fluid storage tanks and gas production units containment structure. See photos. I left the site at 13:33.
2825256 Primary Facility 2019-01-10 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:33; the weather was ~20°F, very windy, and lightly snowing. There were several patches of ice on the pad surface.

There were 3 properly tagged Marcellus wells staged for production. All 3 well cellars were filled with gravel. There were two 550-gallon plastic tanks plumbed to the wells without additional secondary containment. There were 3 GPUs and 3 produced water tanks onsite and in secondary containment.

The site was greater than 70% vegetative cover and no E&S issues were noted. Two infiltration basins were installed along the western edge of the pad. Both basins were about halfway full with ice at the time of inspection.

No violations were noted at the time of inspection.
