2597430 Primary Facility 2017-05-24 Routine/Complete Inspection No Violations Noted I arrived onsite @ 12:05 hrs. and met Ray Miller and Brandon Jessen of Chief. H& P Flex rig 513 on location drilling the South 1H well; crew was nippling up the well for a BOP test when I arrived. Sitewide liner was covered by rig mats, berms appeared to be in good condition. Some rusty colored water had collected in a corner of the containment near the chemical storage area where no rig mats were down; I recommended it be vacced up. Generator sets are being run with natural gas brought in from pipeline. I departed @ 12:50 hrs.
2608496 Primary Facility 2017-06-15 Follow-up Inspection No Violations Noted Arrived @ 12:40. Weather:cloudy, cooler. HP 513 was onsite drilling the SGL 12 B North Unit 1H production hole at 11, 486 at the time of my inspection. There were also three recently TD’ed wells accessible in light of the rig operations. These wells were:

SGL 12 B N Unit 3H SGL 12 B S Unit 1H SGL 12 B S Unit 3H. Accompanying me was Jon Hafer (Chief) who brought with him a fresh water truck and a vac truck. Met with Company Man Trey Villeneuve. The purpose of this inspection was to fill the cellars of the TD’ed wells, to determine whether there was methane outside of surface casing. Water was added to each cellar to a level just above the wellhead flutes, where the cement between the conductor and surface casing was exposed. Following the addition of the water, we waited approximately 20 minutes and searched for any signs of bubbling. No signs of bubbling were seen in the cellars of the 3 wells listed above. Following these observations, the vac truck removed the water from the cellars. The result of

2626309 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted Arrived @ 9:40. Weather: sunny. There were 4 wells on site, all with frac heads.

SGL12 B S Unit 3H – cellar empty, cement bottom visible. Only 9-5/8” x 5-1/2” annulus vent open to polytank.13-3/8” x 9-5/8” annulus appeared to be open, the welded cap across the annulus appeared to be partially off. SGL 12 B N Unit 3H – GE RSH2 wellhead, gravel in cellar with water. Water level approximately 2” above the flutes, no bubbling. 5-1/2” 2000 psi (0-10,000 psi analog). 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annuli plumbed to a polytank shared with the SGL12 B S Unit 3. 80% methane/volume at polytank opening (Altair 5X gas meter calibrated on 8-14-17). SGL 12 B S Unit 1H – GE RSH2 wellhead. Water and gravel in cellar, can’t see flutes, no bubbling. 5-1/2”:2000 psi (0-10,000 psi analog). 13-3/8” x9-5/8” and 9-5/8” x 5-1/2” annuli plumbed to polytank . SGL 12 B N Unit 1H – GE RSH2 wellhead. Flutes visible water and gravel in cellar, no bubbles. 5-1/2”; 2000 psi (0-10,000 psi, analog). Both annuli plumbed to polytank

2656924 Primary Facility 2017-10-31 Routine/Complete Inspection No Violations Noted Site was quiet when I arrived, with only 2 vac trucks and their operators on location. 4 temporary sand separators were set up on containment near the well heads. The secondary containments for the Baker tanks were in excellent condition, and the sitewide liner also appeared to be in good shape. Some foam berms had been removed from the sitewide liner in a traffic area, but no fluid was present on or off containment that showed elevated conductance. I recommend that these berms be left in place as much as possible.

2813049 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Four wells are staged for production. The well tag for the South 1H well has fallen off and is sitting next to the well head. An unlabeled container is present near the well heads. The department recommends labeling the container.

Four steel produced water tanks are present and in good condition. These sit within steel containment with spay-in liner in good condition. The water within the containment was frozen at the time of inspection. Four GPUs are present within containment. The water within the containment was unfrozen and had a conductivity of less than 100 uS/cm.

Two infiltration areas appear to be built as permitted and functioning as designed. Compost filter sock is deployed as the E&S plan calls for. The slope near the northwest corner of the pad is rocked. There is a series of rills in the rock here. Some of the rock appears to have slid down this slope as well. The department recommends inspecting this slope frequently to prevent
