2623449 Primary Facility 2017-08-08 Routine/Complete Inspection No Violations Noted I met Floyd Roberts and Eric McClure of Chief when I arrived. A coil tubing rig was on site drilling out plugs on the 2H well. Puddles were widespread on the site. Small puddles of 6.1 and 8.3 mS/cm were present on containment near the Baker tank battery; puddles on the pad nearby showed low conductance (.45-.76 mS/cm). On the far side of the pad from the entrance, I found 3 small volume puddles (all less than 5 gallons) near the edge of the liner that showed conductance of 2.2-2.4 mS/cm; other puddles nearby showed low conductance (.48-.87 mS/cm). I informed Eric and Floyd, who had been joined by Gib Blevins of Chief; Gib immediately got a vac truck that was on location to vac these areas.
2655236 Primary Facility 2017-11-01 Routine/Complete Inspection No Violations Noted Arrived at 12:00. Weather: cold rain. 2 producing wells on site. Annuli for both wells are plumbed into one 550-gallon poly tank. There is a small amount of oil-base and water in the tank. Monitoring with an Altair 5X gas meter (calibrated on 11/1/17) gives 10 % methane/ volume at the tank opening. NO combustible gas was detected at the 13-3/8" x 9-5/8" annuli of either well. No violations were noted during this inspection.
2710599 Primary Facility 2018-03-26 Follow-up Inspection No Violations Noted Arrived at 11:00. Weather: sunny. The purpose of this inspection was to follow-up on a GPU fire which occurred on 3/25. Crew and staff were onsite. Among the personnel present at the time of my inspection were: Sean Van Fleet (DEP), Ray Miller (Chief-Safety), and Greg Roher (Chief-Completions). Peg Farrell (Chief) was also in phone contact during the inspection. There were 2 gas wells capable of production on location. These were shut-in at the time of my inspection. Two new GPU¿s (Energy Weldfab) were being set up on new containment when I arrived. The large shut-off valve at the front of the 2H GPU washed out (see photos), providing the gas source for the fire. The 2H well did not automatically shut-in. Pressure in the above ground pipe behind the GPU ruptured the pipe. The fire destroyed the communications cables so that it was not possible to shut-in the 2H from Chief¿s control center. Ray Miller arrived on site approximately 30 minutes after the problem was identified and shut-in the 2H well manually.
2710767 Primary Facility 2018-03-26 Incident- Response to Accident or Event No Violations Noted I arrived onsite to follow up on a reported incident. DEP was notified by PEMA at 07:00 hrs. on 3/25 that one of the GPUs on the site caught fire, damaging both GPUs. When I arrived, Cuthbertson Construction Inc. was on the site; the damaged GPUs had both been removed and replaced, and Cuthbertson was hooking up the new units. I met Darren Mundrick of Chief; as he is with Chief's construction group, he was not privy to all of the details of the incident. At that time, Rebecca Renner, OGI for the DEP Oil & Gas program arrived. In order to get us details regarding the incident, Darren called Peg Farrell, and Becky and I both spoke to her and Tyler Berkey about the incident. Ray Miller of Chief arrived while Becky was speaking to Peg; he informed me that the fire and explosion had been reported to 911 at 06:00 hrs. on Sunday morning, and he arrived at 06:30 and secured the site. I then spoke to Peg and Tyler. They told me that once the fire was out and the equipment cooled off, the glycol was vacced out
2845011 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted On February 21, 2019, Department representative Brad Kline conducted a routine site inspection at Chief Oil & Gas LLC. Jacobson Pad. Weekly inspection log was onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

There are a total of 2 producing wells located on this well pad. All wells are tagged with API/well permit numbers. Cellars are filled in with gravel. There are 2 GPU’s and 2 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted.
