The site is surveyed and staked. The 18 inch silt sock is being installed around the perimeter of the LOD as designed on the site construction plans. No obvious wetlands (based on vegetation and soil saturation) noticed within the LOD. The bulk of the earth disturbance will begin on May 31st, 2016. The site may require extensive blasting for pad construction. Keith Kimball with Chief Oil and Gas was on site to discuss the pad construction with the DEP. No violations noted.

2549195 Primary Facility 2016-12-22 Routine/Complete Inspection No Violations Noted I arrived at the site at 13:00. There was no activity or Chief Oil and Gas personnel present during the inspection. The pad has been constructed. Chief submitted a spud notice to the Department on 12/21/16 at 8:25 AM. The notice indicated the spud date for the Martino 1H well is 12/23/16. The sign on the lease road lists the Martino 1H well, which indicates that this is the only, permitted well on the pad. There is a containment liner with berms on all (4) sides located in the center of the pad. There are 2 wooden stakes with orange flags on top of them in the containment. There are orange spray painted markings and the word "Pipe" on the berms of the containment. There is a red colored container located next to the containment. The container is mainly empty. There is a small amount of water, some of it frozen, and some dirt and rocks in the bottom of the container. It appears as if either Chief has recently spud the 1H well or is in preparation to do so. 2563367 Primary Facility 2017-02-21 Drilling/Alteration No Violations Noted I arrived at the location at 11:30. Weather conditions are partly sunny and breezy with the temperature around 40 degrees. The site is a bit muddy due to recent snow and ice melt. Helmerich and Payne (H&P) Flex Drilling Rig # 513 is on this location. The mailbox on the access road contains waste manifests for drill cuttings which are transported to Phoenix Resources Landfill in Wellsboro, PA for disposal. The Preparedness, Prevention And Contingency Plan is dated May 2016. Also located in the mailbox is Drillers Log, Weekly Containment Inspection Form, Emergency Response Plan, Weekly Erosion And Sedimentation Log, permit for 1H well, ESCGP 2 and Construction Permit. The current drilling depth on the 1H is (4,712) feet. The Chief Incident Contact Information Short Form, Drilling Contact List, Incident Protocol and 1H Well Permit are posted in the company man trailer. Frack operations were being prepared on the site. Frequent water truck deliveries were occurring during the inspection. An open freshwater modular storage vessel (~640,000 gallon) was being filled for fracking use. All regulated substances were contained within secondary containment. No leaks detected. No violations noted.

2581167 Primary Facility 2017-04-11 Routine/Complete Inspection No Violations Noted Flow back operations for both fracked wells are in process. Containment around well heads still in place. Slightly Conductive (1500 mS/cm) water was detected under containment near wellhead. Multiple baker tanks remain on location for flow back operations and are within secondary containment. No leaks from tanks detected.
2582841 Primary Facility 2017-04-10 Drilling/Alteration No Violations Noted Arrived at the site at 13:20. As I was signing in to the site I engaged in a brief conversation with the site security guard. He stated that flowback is in progress and it took 4 days to frac a total of 37 stages for both wells and one day to drill out frac plugs in the wells. Chief submitted stimulation notifications to the Department on 3/27/17. The notifications indicated that Chief would commence stimulation (hydraulic fracturing) of the Martino Unit 1H and 2H wells on 3/30/17 at 6:00 AM. I met with Mason Hope, Well Tester, of Oil States Energy Services, the subcontractor of Chief in the process of conducting flowback operations on the Martino Unit 1H and 2H wells. Mr. Hope informed me the following in reference to site activities. "Flowback operations commenced on 4/7/17. Natural gas is being sent into the pipeline to sales. Flowback fluid flows up the wells to the surface, then through the sand traps , then the flowback separator units and eventually into iron flow lines into the first row of frac
2608916 Primary Facility 2017-06-28 Complaint Inspection No Violations Noted Two Wells in production status. Two each dehydration units within felt and plastic lined containment. No leaks detected from production equipment. No spills or leaks detected on pad surface. Site stable. No violations noted. No samples collected. All field conductivity measurements collected with PCTestr 35 multi-parameter meter. This meter is for informational purposes only and was used to indicate if further follow-up (ie. water sample collection) was needed and was not intented to be used as a scientific test instrument.
2609931 Primary Facility 2017-06-26 Routine/Complete Inspection No Violations Noted Arrived at the site at 12:30. The weather conditions were partly cloudy skies and breezy with temperatures in the upper 60's to around 70 degrees. There was no activity at the site. I met with Kellon Balum, Environmental Field Coordinator of Chief Oil and Gas during the inspection. The 1H and 2H wells have been in production for about a month now and all frac tanks in containment were moved off the pad about a week ago according to Mr. Balum. I did not record any elevated specific conductance (electrical conductivity) readings of concern in water and damp soil on the pad surface and water in the production containment. All conductivity readings were less than (1,000) micro Siemens/cm which is equivalent to (1) milli Siemens/cm. The majority of the readings were less than (500) micro Siemens/cm. I departed from the site at 13:45.
2621548 Primary Facility 2017-07-14 Routine/Complete Inspection No Violations Noted Arrived at 14.10. Weather: cloudy, following rain. There were no personnel on site at the time of my inspection. There were 2 producing wells at this location: the Martino 1H and 2H.

Martino 1H and 2H, water in the cellars, above the well head flutes, no bubbles. Martino 1H:Top gauge: 1800 psi (0-5000 psi analog) 5-1/2”: 1800 psi (0-5000 psi analog) 13-3/8” x 9-5/8”: 8% LEL combustible gas/volume (Altair 5X calibrated on 7/11/17). 9-5/8” x 5-1/2”: couldn’t monitor vent. The result of this inspection was NOVIO, because no violations were observed at the time of the inspection. Left @ 15:00

2625665 Primary Facility 2017-08-14 Routine/Complete Inspection No Violations Noted Arrived at 14.45. Weather: cloudy, humid. There were no personnel on site at the time of my inspection. There were 2 producing wells at this location: the Martino 1H and 2H. There were no well tags on these producing wells, notified Peg Farrell.

Martino 1H and 2H, water in the cellars, no bubbles. Martino 1H: Top gauge: 1650 psi (0-5000 psi analog) 5-1/2”: 1650 psi (0-5000 psi analog) 13-3/8” x 9-5/8”: 10% LEL combustible gas/volume (Altair 5X calibrated on 8/14/17). 9-5/8” x 5-1/2”: couldn’t monitor vent. The result of this inspection was NOVIO, because no violations were noted at the time of the inspection. Left @ 15:20

2852242 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:31; the weather was ~35°F, clear, and windy. The pad surface was ice covered.

There were 2 properly tagged Marcellus wells staged for production. Both well cellars were filled with rock. There were 2 GPUs, 2 produced water tanks, and 1 vent tank onsite and in secondary containment. The produced water tank containment was about half full of ice (Photo #3).

There were 2 infiltration basins constructed. Both basins contained large amounts of ice (Photos 2, 4). The site was well vegetated and the steeper slopes were stabilized with rock. No E&S issues were noted.

The Department recommends monitoring the basins as the weather improves to ensure that they are functioning as designed. It is also recommended that excess water be removed from secondary containment as necessary.
