2617793 | Primary Facility | 2017-07-21 | Routine/Complete Inspection | No Violations Noted | One well shut-in. Three total Well cellars. No production equipment.
Pipeline run to location Infiltration basins with spillways recently installed. Wetland greater than 1 acre less than 100 feet from limit of disturbance. |
2647081 | Primary Facility | 2017-10-13 | Routine/Complete Inspection | No Violations Noted | ANNULUS VENT TEST RESULTS
2" Vent (Production x Intermediate) = 0% LEL and 0% Methane/Volume. 1.5" Vent (Intermediate x Surface) = N/A 1" Vent (Surface x Conductor) = N/A Remediation work should be done on plumbing, valves and well head due to excessive rust/corrosion. Possible integrity issues with the above mentioned items. |
2659893 | Primary Facility | 2017-11-09 | Routine/Complete Inspection | No Violations Noted | I met Jeremy Boyd when I arrived. Precision rig 569 on location, just finishing drilling the 12 1/4" section of the Hart North 5H. Containment was in fairly good shape, although I pointed out 2 areas to Jeremy where the liner was torn on the berms. Jeremy told me he would address it. Overall, site appeared to be clean and well maintained. |
2670948 | Primary Facility | 2017-12-12 | Routine/Complete Inspection | No Violations Noted | Precision rig on location. Pipe is being pulled from the 1H well at this time. Six wells drilled at this site. No leaks from containment. No leaks detected from equipment. |
2694790 | Primary Facility | 2018-02-14 | Incident- Response to Accident or Event | Violation(s) Noted | Chief Oil and Gas representative on site: Chris Marsh
Universal conducting the frack. PPC site specific plan on site. The operator reported approximately 200 gallons of fracking fluid escaped at high pressure from an iron fitting attached to the well head. The high pressure fluid cut through the primary containment. The iron fitting was replaced and the containment was repaired. No clean-up under containment. A Vac truck was used were free liquid could be removed. Spilled material appears to have stayed on pad surface. Clean-up to begin after the frack is completed. Violations noted. No samples collected. |
2704575 | Primary Facility | 2018-03-13 | Follow-up Inspection | Outstanding Violations - Viols Req'd | Woodward and Curran on location taking samples of soil that may of been affected from the 2/13/2018 reported spill. All spilled material and soil appears to have been excavated (150 tons) and placed with containers. Conductivity and pH were a respectable 260 uS/cm and 7.03 pH units. Backfill operations will begin once all pipeline supply lines are finished with installation. Referred to DEP Environmental Clean-ups program for further follow-up. |
2706612 | Primary Facility | 2018-03-15 | Routine/Complete Inspection | Outstanding Violations - Viols Req'd | Site was very active when I arrived; company man Gibb Blevins was not at the site when I arrived. Frac was finished, and crews were setting up temporary separators and dehy units, as well as breaking down Baker tank containments. Many puddles were present on the pad surface, with conductance ranging from .56 to 1.3 mS/cm for the most part. Some small puddles (.5-2 gallons) were present near one of the Baker tank batteries that had slightly higher conductance (2-2.5 mS/cm); other puddles in the immediate area were in the range of .8-1.1 mS/cm. I was unable to access the center of the pad due to activity and truck traffic. |
2742659 | Primary Facility | 2018-06-14 | Administrative/File Review | No Violations Noted | A Statewide Health Final Report was approved by the DEP (Environmental Cleanups Program) on 6/14/18. A copy of the approval letter has been uploaded to eFACTS. |
