After TD’ing the 4H, Orion 15 plans to TD the 6H.

This inspection was prompted by an Inactive Status Application for the SGL 12A 5H. At the time of my inspection, Oil States representatives were onsite overseeing flowback operations. Oil States representative Larry Patrick informed me that operations would most likely require a few more weeks. The SGL12A 5H well was covered by containment, so it was not possible conduct an inspection. Left location at 14:20.
2511310 Primary Facility 2016-08-16 Routine/Complete Inspection No Violations Noted Arrived @ 13:00. Weather: hot, humid, rain off and on. This inspection was prompted by an Inactive Status Application for the SGL 12A 5H. At the time of my inspection, flow back operations had been completed. Oil States staff were onsite monitoring a sand separator and heating unit that were staged near the well head so that high initial pressure could be dropped back and sent to the production units on site. As a consequence, the SGL12A 5H well was still covered by containment, so it was not possible conduct an inspection. The status of the other wells on location was as follows:

SGL 12 A 1H – shut in SGL 12A 4H – in production SGL 12 A 2H in production SGL12A 5H – covered by containment SGL 12 A 3H in production SGL 12A 6H – in production. Left location at 14:00.

2531431 Primary Facility 2016-10-07 Routine/Complete Inspection No Violations Noted Arrived @ 9:00. Weather: cool, sunny. This inspection was prompted by an Inactive Status Application for the SGL 12A 5H. This inspection could not be completed at this time because Oil States staff were still using a heating unit on the 6H and the 5H was still covered by containment. The status of the other wells on location was as follows:

SGL 12 A 1H, shut-in,not producing. Gravel and water in cellar, no bubbling. 9-5/8” x 5-1/2”: 1200 psi (analog gauge) –queried this with Jon Hafer. SGL 12A 3H, in production, 5-1/2”: 1100 psi (0-5000) SGL 12A 4H, in production, 5-1/2” 900 psi SGL 12A 5H, covered by containment SGL 12A 6H, Top gauge: 2125 psi (0-5000, analog), 5-1/2” 2125 psi. There were no violations noted at the time of this inspection. Left location @ 11:15

2552280 Primary Facility 2017-01-13 Routine/Complete Inspection No Violations Noted Five wells in production stage. One will drilled but not in production. Four water separator's appear to be in use and within containment. Four production tanks within containment. No violations noted.

2559026 Primary Facility 2017-01-31 Routine/Complete Inspection No Violations Noted Arrived @ 10:00. Weather: cold, snowing. Met with Jon Hafer (Chief) to conduct inspections for Inactive Status Applications for the SGL 12A 5H and 1H. Observed Hafer as he monitored annular vents for combustible gas (Multirae meter calibrated 1/31/17).

SGL 12A 1H, drilled to total depth, gravel in cellar. 13-3/8” x 9-5/8”: 0.3% methane 9-5/8” x 5-1/2”: 100% methane SGL 12A 5H: to, top-holed to 1953’ 13-3/8” x 9-5/8”: no methane detected 9-5/8”: no methane detected. SGL 12A 2H, SGL 12 A 3H, SGL 12A 4H, SGL 12A 6H are in production. Left @ 11:30. No violations observed during this inspection. Approval of the Inactive Status Applications is recommended.

2747140 Primary Facility 2018-06-29 Routine/Complete Inspection No Violations Noted Fours wells staged for production phase. Four each water separators. Dump lines wrapped in insulating blanket. No leaks detected from production equipment. Above ground fresh water storage tanks in use on location. No violations noted.
2813321 Primary Facility 2018-12-06 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. There are 5 well heads. Four of the wells are stages for production. Well tag for the 5H well has fallen off of the well head. An unlabeled container is present near the well heads. The department recommends labeling the container.

Four steel produced water tanks are present and in good condition. These sit within steel containment with spay-in liner in good condition. The water in the containment was frozen at the time of inspection. Four GPUs are present within containment. The water within the containment was unfrozen and had a conductivity of less than 100 uS/cm.

Two infiltration areas appear to be built as permitted and functioning as designed. Site is stable. No E&S issues noted.
