The site sign, drilling permits, weekly E&S reports, and emergency response plan were onsite for review.  The E&S inspection log was reviewed; the last routine inspection was conducted on 03/18/2019. With exception to the area below Infiltration basin 2 (Photo#1), no erosion issues were observed. The site is stable and well vegetated meeting the 70% uniform, perennial, vegetative cover.

A total of one producing and one plugged well is located on this well pad. All wells are tagged with API/well permit numbers. The well cellar was filled with gravel. There is one GPU and one steel vertical production fluids tanks onsite. The GPU and production fluid tank were stationed in secondary containment. PCSMs have been installed and functioning well.

The Department recommends:
