2723527 Primary Facility 2018-04-25 Incident- Response to Accident or Event No Violations Noted The Department of Environmental Protection conducted an incident inspection at the EOG Resources – Crane 1H & 2H Well Pad located in Smithfield Twp, Bradford Co. on 4/25/2018 @1130 hours. Present for the inspection was Nathan Wells, EOG Safety and Environmental Specialist. Slumberger is currently fracking the 1H well. The frack was temporarily shut down to enable cleanup of the spill.

On 4/25/2018 @0430 hours a spill was called in to the Department regarding a tote of Friction Reducer (FR) that was dropped while being unloaded. The ruptured container released approximately 300 gallons of fluid onto the southeastern edge of the pad. EOG estimates approximately half of the FR remained on the pad, while the other ~150 gallons spilled over the edge of the slope and proceeded to sheet flow down into the E&S channel at the base of the slope. The FR ran down the channel into a rip rap lined storm water infiltration basin. EOG used vacuum trucks to recover as much of the FR fluid as possible, created dykes to cont

2761373 Primary Facility 2018-08-07 Administrative/File Review Violation(s) Noted On June 4, 2018, EOG Resources, Inc. (EOG) submitted a completion report for the Crane 2H gas well. The completion report documented the use of 1,884,560 gallons of water from Sugar Creek (Isbell) operated by Chesapeake Appalachia, LLC. This source was previously authorized under the Water Management Plan, but the authorization expired September 30, 2017 and was not renewed. Information obtained via email from the Susquehanna River Basin Commission (SRBC) confirmed that unauthorized withdrawals had taken place from May through June 30, 2018, as reported by EOG to SRBC.

EOG failed to obtain authorization under an approved Water Management Plan to utilize Sugar Creek (Isbell) as a source of water for drilling or hydraulic fracture stimulation for the Crane 2H gas well, which constitutes violations of 25 Pa. Code § 78a.69(a)(1) and Section 3211(m)(1) of the 2012 Oil & Gas Act, 58 Pa.C.S. § 3211(m)(1).
