2836167 Primary Facility 2019-02-04 Follow-up Inspection No Violations Noted Follow up inspection conducted in response to a spill notification from the operator on 1/19/19. A spill of produced water reported by operator on 1/19/19. Operator estimated the release to be approximately 20 gallons. The produced water was reported to have impacted the gravel on the pad.

One roll-off container, lined with plastic, is present on the pad near the produced water tanks. Puddles nearby on the pad have conductivity readings of around 51-52 uS/cm. No excavated areas were noted on the pad. The pad was snow and ice covered so any excavated areas may have been concealed.

Three wells are staged for production. No well tags are present on the well heads. The Department recommends labeling the well heads with well tags. Well cellars are filled with gravel.

A large white unlabeled plastic container is present within secondary containment near the well heads. The Department recommends labeling thus container. GPUs are present without secondary containment. GPUs show some moderate corrosion. Two dou
