2278561 Primary Facility 2014-05-22 Routine/Complete Inspection Violation(s) Noted VIOL 78.73(b) - failure to prevent gas below casing seat from entering fresh gw; VIOL 78.81(2) - failure to prevent migration of gas into fresh gw; VIOL 78.85(a)(5) - failure to prevent gas flow in the annulus; VIOL 78.86 - failure to report defective cement.
2295125 Primary Facility 2014-08-13 Site Restoration No Violations Noted Drilled, but not in production. Restoration Report and NOT submitted
2404560 Primary Facility 2015-07-29 Drilling/Alteration Outstanding Violations - No Viols Req'd T&A logging on 2H underway at time of inspection.
2445541 Primary Facility 2016-01-27 Drilling/Alteration Outstanding Violations - No Viols Req'd Arrived onsite @ 9:00. Weather: sunny, cool. There were two wells on location. Both were drilled to total depth and toe-fracced. Issues concerning defective cement prompted previous diagnostic and remediation efforts and at the time of my inspection both wells were being flared. I met with EOG representatives Chuck Peterman and Russ Juart who have been overseeing the flaring which began on 12/31/15.

Oberkamper 1H: there was tubing in the well with a packer below the previously perf-and-squeezed zones. They were running gas through the production units to capture any sand and water and the ABB total flow meters to quantify the gas prior to flaring. Since flaring began flowing pressures have dropped from initial shut-in pressures of ¿ 2500 psi to50 - 60 psi. EOG is monitoring annular pressures and conducting 72-hour PBTs on a regular basis, but has not yet analyzed data. Violations for defective cement were cited on 5/22/14 (2278561). There were no new violations so the IR Result is OUTNO.

2505551 Primary Facility 2016-07-28 Routine/Partial Inspection No Violations Noted 2 Marcellus Shale wells w/ 3 annulus vents each.
2555501 Primary Facility 2017-01-25 Routine/Partial Inspection No Violations Noted 2 Marcellus Shale wells on a ~0.4 acre pad. Liner around well heads holding water above and below liner. Ice accumulated around well heads, so I did not get close enough to see if there are well ID tags
2696483 Primary Facility 2018-02-21 Routine/Complete Inspection Outstanding Violations - Viols Req'd Unable to check annulus vents due to unsafe conditions, Can not tell where the cellars of the wells are due to containment floating as well as deep standing water on top of containment. All three of the annulus vents are vented to 55 gallon polly drums on location.

    • The violation code for 25 Pa. Code § 78.81(a)(2) was updated on May 6, 2019 because this violation code was not available in eFACTS when this inspection record was created. This violation was previously listed as §78.81D2, which was the closest violation code available in eFACTS to the actual violation at the time. Although this is an unconventional gas well currently subject to the Rules and Regulations of 25 Pa. Code §78a, the violation was originally cited in 2014 and the §78a regulations were not effective until October 2016. Therefore, to avoid confusion, the outstanding violation will remain as initially cited (§78.81(a)(2) instead of §78a.81(a)(2)) until it is either superseded or resolved. ** ~CMB~
