The well was never completed and has been used as a monitoring well for unconventional well development in the area. The rig is off location following final plug and abandonment. At the time of inspection, all casing is left in the hole and there is a conventional wellhead in place. The cellar was full of fluid; no bubbling was observed. There were 3 annular vents; no audible or visible discharge was noted. No obvious issues were observed at this time.

2817667 Primary Facility 2018-12-19 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging inspection of the Guinan 1V was conducted December 17, 2018. Plugging was completed on November 12, 2018 and a certificate of well plugging has been received. At the time of inspection, the well has a conventional wellhead still in place; production casing is shut-in and there were three annular vents. The certificate of plugging references a candy cane cent as a plugged well monument. I spoke with the operator; welder is delayed and will install monument when available. No obvious major issues were observed at this time.
2824173 Primary Facility 2019-01-02 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, post-plugging, follow-up inspection of this recently plugged well was conducted January 2, 2019. At the time of inspection, two 2" vent pipes have been installed on the existing conventional wellhead along with a marker plate as a plugged well monument. The the yellow vent pipe with the marker plate is the surface casing vent, while the other vent pipe is for production casing. There was no visible or audible venting observed. The location was wet, and the immediate area surrounding the base of the monument had standing water. No bubbling was observed. No open holes, discharge, or other obvious issues were observed at this time.
2850265 Primary Facility 2019-03-07 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine, follow-up inspection of this plugged vertical well was conducted March 7, 2019. At the time of inspection, the location was snow-covered. Two vent pipes have been installed on the well along with marker as a plugged well monument. One vent is for production casing and the other is for the annulus. There was no audible or visible venting or discharge observed. The area surrounding the base of the casing and vent was frozen and snow-covered; no bubbling or evidence of discharge was observed. No obvious issues were observed during this inspection.
