1805988 Primary Facility 2009-06-09 Drilling/Alteration De minimus Violations Noted Site was inspected with O&G inspector Bruce Jankura. Site ID sign and E&S plan are on site. BMPs are in place. On back edge of site, there was an area of accelerated Erosion about 1ft wide by 4ft long across fill slope. The material washed out the wood chip berm and carried out into the woods about 6ft. Issue was discussed with Dave Bimber on site and proper clean-up and restabilization was addressed. There were no other E&S control issues on site. There were a few small piles of KCl near chemical storage area. Piles were about six inches in diameter. These will also be cleaned up as soon as possible. Pit liner was sound and liquid level was greater than 2ft. of freeboard. Truck was on site to haul off some of the waste water. Waste water is being disposed of properly at an Altoona wate facility
1811606 Primary Facility 2009-07-07 Follow-up Inspection No Violations Noted On back edge of site, where area of accelerated Erosion was previously noted, all material had been pulled back onto site and area was seeded and mulched. The wood chip barrier was replaced and the hill had been reconstructed and stabilized. There was some build-up of sediments along wood chip berm that Mr. Anderson Addressed. It will be cleaned out immediately. On day of inspection rig was being moved off site. There were no other E&S control issues on site. The waste water from the pit is being transferred to sewage treatment plants in Altoona and Johnstown. So far 394,700 gallons have gone for disposal. The drill cuttings are being solidified with cement and hauled to Sandy Run Landfill in Hopewell, Pa Pit liner was sound and liquid level was greater than 2ft. of freeboard. Neighboring landowner was observing and documenting inspection from 4-wheeler parking at edge of site and camoflauged Turkey Blind.

No violations are noted at time of inspection.
