This well is an unconventional well and was reported to be plugged on 5/7/2018. Per current regulations, the post-plugging restoration report shall be made electronically on forms provided by the Department through the Department’s web site. As this restoration report was incorrectly submitted, it is not being approved.

No violations are being noted at this time as the operator is within the regulatory requirements for submission of the post-plugging restoration report.

2836029 Primary Facility 2019-02-04 Site Restoration No Violations Noted On 2/4/19 at 1109 hrs., I conducted a restoration inspection of EQT Production Co.'s Century Hill well site in response to a Post-Plugging Restoration Report submitted for well permit 005-31228 by EQT on 1/21/19 (attached). The well site is located in West Franklin Twp., Armstrong County. A review of previous Department inspections show that a paper submittal of this report was initially rejected due to the fact that the operator is required to submit the report electronically. Previous Department inspections also reflect that the ESCGP associated with the site has been terminated. Upon inspection, it was confirmed that the well site has been restored and is permanently stabilized according to the as-built schematic (attached) which was also submitted with the restoration report. The restoration report is hereby approved; however the operator should remove the well permit signs, Act 9 site ID sign, and permit mailbox from the entrance to the location. There are no violations noted for this inspection.
