Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2200198 Primary Facility 2013-08-22 Follow-up Inspection Violation(s) Noted Orginal permit 005-30785 expired on 2/11/13. Repermitted (005-31136). Well site n AR constructed except for stream crossing of UNT to Brady Run, which would reguire a GP. Previously constructed waterbars have eroded and discharging sediment offsite. Well pad has not been temporarily stabilized, resulting in accerlated erosion and sedimentation offsite after overtopping temporary bmps (ie filter fence).
2217867 Primary Facility 2013-10-31 Compliance Evaluation No Violations Noted The permit 005-30785 expired on 02/11/13. The new permit number is 005-31136. Well-site and access road had been constructed except for a stream crossing of an Unamed Tributary (UNT) to Brady Run. The operator had addressed the issues with erosion controls and erosion as outlined in the previous inspection. At this time there is no observed accelerated erosion or sedimentation. The Department does recognize that if the site is left in its current condition throughout the winter months and is not further developed and subsequently restored the previous erosion issues are likely to occur again.
