Site was seeded and mulched. Incipient erosion trails were evident which, if left untreated, could result in severe accelerated erosion. Department recommends the operator closely monitor the situation and take appropriate measures to preclude accelerated erosion and establish 70% uniform vegetative cover.

2480931 Primary Facility 2016-05-02 Routine/Complete Inspection No Violations Noted Well tagged-in production-equipment ok. Responded to a leaking gas call. Unplugged 1/2" ball valve handle on meter run had been bumped open. Operator closed valve handle-removed handle from valve stem and installed a 1/2" plug in valve to prevent any possible future release of gas from valve. Requested that Operator check all wells to ensure that valves had plugs installed in them.
2727939 Primary Facility 2018-05-09 Plugging(Includes Plugged/Mined Through) No Violations Noted The operator had moved a plugging Rig in and had it set up over the well at the time of this inspection. The tubing was also on location to run in the well bore and cement through after establishing the approved attainable TD. All equipment on site looked ok.
2729513 Primary Facility 2018-05-14 Plugging(Includes Plugged/Mined Through) No Violations Noted The plugging operator had the well open to attempt to blow it down at the time of this inspection. The well was blowing steady gas so the rig crew called for 50 bbl of fresh water and was waiting on their water to arrive so they could pump it down the well to load the hole and attempt to kill the well so they could set their bottom cement plug. All equipment on site looked ok.
2730861 Primary Facility 2018-05-17 Plugging(Includes Plugged/Mined Through) No Violations Noted The crew ran the bottom cement plug from 6478' to 4550'. They then shot the 4 1/2" pipe off at 4300', could not pull the pipe after exerting over 160% of pulling pressure, so they perforated the pipe with 4 shots every 100' clear up to 2930' and shot off the pipe at 2930'. The Rig was running pipe back in the hole to attempt to pull the rest of the 4 1/2" pipe at the time of my inspection.
2734311 Primary Facility 2018-05-23 Complaint Inspection No Violations Noted At the time of this inspection, plugging of the well appears to have been completed.

Landowner and operator representative Randy Morris arrived on-site at approximately 1110.
