1985951 Primary Facility 2011-07-06 Routine/Complete Inspection No Violations Noted Site had been constructed, drilling had not commenced. Noted that the collection ditch at the bottom of the well-site was not installed as per the E&S plan rendering. The outlet end of the ditch had silt fencing utilized improperly installed as an outlet protectio9n. The ditch when used on this site should have a rock filter outlet or sediment trap. In addition the runoff should not be directed towards the cultivated corn field. The access road is shared with an existing farm road and has minimal erosion controls. The drill pit also required subbase material as outlined in the 78.56(a)4ii. The pit should be addressed prior to installing the liner. Spoke with Wilmoth Interests on 07/07/11 regarding these two minor issues and was informed that the pit will be lined with road fabric and that the interceptor ditch will be addressed.
1995775 Primary Facility 2011-08-12 Routine/Complete Inspection No Violations Noted Drilling had been completed, drill pit had been backfilled, and site had been reclaimed. Vegetation had begun to grow. Well appeared to be equipped for production. No accelerated erosion or sedimentation was observed in relation to the site or access road.
2076666 Primary Facility 2012-06-15 Site Restoration No Violations Noted Well appeared to be equipped for production, brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation observed in relation to the access road or well-site. Vegetative cover met the 70% uniform coverage criteria.
