Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1914197 Primary Facility 2010-09-17 Drilling/Alteration No Violations Noted
1925082 Primary Facility 2010-10-28 Routine/Complete Inspection No Violations Noted Well is drilled but not equipped. Site ID and E&S plan located. Pit is on-site / in sound condition. Erosion and sedimentation controls are sufficient however there is potential for sediment to enter the adjacent field. It is recommended that silt fence is added between the well pad and the field.
1984176 Primary Facility 2011-06-16 Routine/Complete Inspection No Violations Noted Well operating and properly identified.
2090110 Primary Facility 2012-08-08 Site Restoration No Violations Noted At the time of inspection the well appeared to drilled and equipped for production. The associated well apparatus and brine tank located onsite appear to be in sound condition. No observation of accelerated erosion or sedimentation in relation to the well pad or access road. The site appears to be restored and meets the 70% uniform vegetative coverage criteria.
2185997 Primary Facility 2013-07-23 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Site had been restored properly and there was no observed accelerated erosion or sedimentation. There was minor erosion observed with the access road as it approaches Church Road. Without action by the operator this could evolve into an erosion issue which transports sediment onto Church Road. At this time the Department is requesting that the operator address the access road issue and forward documentation that this has been completed to the address on page 1 of this inspection and to the attention of Water Quality Specialist Greg Higgins. This documentation should be forwarded no later than August 10, 2013.
2776689 Primary Facility 2018-09-19 Routine/Complete Inspection No Violations Noted This well is tagged and shut in. The annulus is open. The well head casing, piping, equipment and 50 bbl fiberglass fluid collection tank look ok.
