Drill and frac pits located onsite at the time of the inspection and they had more than the required two feet of lined freeboard. Noted that it was raining at the time of the inspection and that all erosion control BMP¿s appeared to be functioning properly. No accelerated erosion & sedimentation was observed in relation to the access road or well-site at the time of the inspection. Noted that it appeared that significant dirt moving and culvert replacement would have had to have been undertaken to make the existing stream crossing for Fagley Run capable of handling the truck traffic necessary for the development of wells located on the well pad. At this time the Department is requesting that the operator forward documentation outlining what work, if any, was performed by the operator to make this stream crossing adequate for well development. This documentation should be forwarded no later than 06/18/10.

No accelerated erosion & sedimentation was observed in relation to the site or access road.

No accelerated erosion & sedimentation was observed in relation to the site or access road.
2007943 Primary Facility 2011-10-04 Complaint Inspection No Violations Noted Well appeared to be equipped for, but not in production at the time of the inspection. Well 005-30588 is also located on the well pad. Drill and frac pits located onsite. The drill pit contained drill cuttings and fluids and the frac pit contained fluids. Both pits had more than the required two feet of lined freeboard, but due to recent precipitation there were areas of the pit which were approaching the two foot threshold. The site can remain as is it is at this time due to the renewal of drilling permit 005-30589. The operator should continue to maintain all erosion control BMP¿s and monitor the drill and frac pits to insure continued compliance with freeboard requirements.

All erosion controls ahd been implemented as outlined in the E&S plan included with the approved ESCGP.
