1934024 Primary Facility 2010-12-14 Routine/Complete Inspection No Violations Noted Drill rig onsite and drilling appeared to be about to commence at the time of the inspection. Below grade pit had been constructed properly but had not as of yet been lined. E&S controls implemented as outlined in the E&S plan. Access road had been heavily rocked to maintain its stability due to its short distance and proximity to SR 1028. Operator should continue to monitor the site and erosion controls to insure their continued functionality until well development activities are completed.
1940339 Primary Facility 2011-01-03 Complaint Inspection No Violations Noted nspection of well site was in response to a water supply complaint of a contaminated spring (Complaint Id#276486). Well was frac'ed during the last week of December. Well site ID and E/S plan were in place. Site was being regraded at time of inspection. Cuttings' pit was encpsulated. Frac pit was reported to be removed along with contents. Silt fence was stressed at the far corner of site but there was no offsite erosion visible.
1944930 Primary Facility 2011-01-25 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed at the time of the inspection. Drill pit that had been located onsite had been regraded. All erosion controls appeared to be intact. Operator should continue to monitor these erosion controls and access road until well development and site reclamation activities are completed.
1961080 Primary Facility 2011-03-23 Routine/Complete Inspection No Violations Noted Inspection was in conjunction with water supply sampling due to contamination event during the last week of December.
2083652 Primary Facility 2012-07-17 Site Restoration No Violations Noted Drilling was completed on 12/17/10. Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Well-site had been regraded. It was noted that the well was located amidst cultivated agriculture fields.
2270679 Primary Facility 2014-05-14 Routine/Complete Inspection No Violations Noted Producing gas. Tag, tank, e&s ok.
2379678 Primary Facility 2015-06-17 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that a cultivated corn field was located immediately upgradient from the well.
