Met onsite with operator representative Walter Emquist regarding the proposed site and E&S plan. Discussed the possibility of attempting during construction to pull the site back up over the top of the hill away from the steep grade leading to North Fork Pine Creek. Mr. Emquist stated that this will be attempted, and if revisions are required on the E&S plan he will contact WQS Higgins. The E&S plan as compiled should effectively minimize accelerated erosion & sedimentation provided the plan is implemented and maintained properly.
1872622 Primary Facility 2010-03-26 Routine/Complete Inspection No Violations Noted Drill rig located onsite but drilling was not ongoing at the time of the inspection. Below grade pit located onsite. It had more than the required two feet of lined freeboard and appeared to be in sound condition. Silt fence backed with hay bales had been implemented properly and the edge of the site had been pulled back onto the flat thus greatly reducing the amount of disturbance neccesary for the site. A small spring and/or runoff flow was noted adjacent to the East side of the access road and well-site. This flow was going over the disturbed earth below the well pad and was building up at the silt fence and slowly going under the fence and hay bales. Contacted operator representative Walt Emquist regarding this matter and suggested that this flow be captured and directed away from the disturbed area. Mr. Emquist stated that he will visit the site today and evaluate the situation.

1875290 Primary Facility 2010-04-07 Compliance Evaluation No Violations Noted Observed that the rock lined erosion control ditch implemented had been extended to discharge to an existing swale. Noted that the pit fluids had been removed so as to restore the two feet of lined freeboard. The operator should continue to monitor the erosion control measures to insure the continued effectiveness until final site restoration activities are completed.
