1818960 Primary Facility 2009-08-10 Routine/Complete Inspection No Violations Noted Site was staked, not constructed at the time of the inspection. The E&S plan as compiled should effectively minimize accelerated erosion & sedimentation provided the plan is implemented & maintained properly.
1895723 Primary Facility 2010-07-02 Routine/Complete Inspection No Violations Noted Site constructed, drilling had not been commenced at the time of the inspection. Below grade pit located onsite. Pit was properly lined. Noted that drains had been implemented to carry any water from a ditch at the bottom of the cutslope to carry spring flow or runoff below the fill slope. Ideally this water should have been carried via the drains to outside of the disturbed acreage of the well site. As constructed the drains should be effective, however they should be monitored and if as constructed they are not functioning properly the piping should be extended beyond the disturbed acreage and outletted either with appropriate BMP's or to a stable area.
1897679 Primary Facility 2010-07-13 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pits located onsite had more than the required two feet of lined freeboard. Tool pusher onsite indicated that runoff flow was compromising the auxillary pit. Specifically the runoff is flowing to the cut side and progressing underneath the pit liner. The Department recommends that the operator not utilize this pit any further and remove the liner. Should an additional auxillary pit be required an above grade pit should be utilized. All erosion controls had functioned properly minimizing erosion & sedimentation. The operator should continue to monitor the site to insure all erosion controls continue to function properly

1953785 Primary Facility 2011-03-02 Routine/Complete Inspection No Violations Noted Met onsite with operator representative Tom Baun to disuss additional measures that are required to address the runoff flow conveyed by culvert under the access road and the runoff flow from the backside of the well pad downslope. WQS Higgins and Mr. Baun agreed that a small sediment trap and rock stabilized ditch on botht he inlet and outlet side of the culvert would be required for permanent stabilization. In addition a erosion control blanket lined (lined until the vegetation takes hold) swale is to be implemented to convey the runoff collected on the backside of the site to the downslope side. WQS Higgins and Mr. Baun agreed this work should be completed by the end of March 2011.
1963441 Primary Facility 2011-04-04 Routine/Complete Inspection No Violations Noted Inspection performed as a follow-up to the 03/02/11 inspection and site meeting with operator representative Tom Baun. The permanent erosion control BMP work that was discussed and agreed upon has not been completed or commenced as of this inspection. Observed no additional erosion and sedimentation issues have developed since the 03/02/11 inspection and meeting. Contacted operator representative Tom Baun who indicated that as soon as weather permits the work will be completed.
1995777 Primary Facility 2011-08-12 Routine/Complete Inspection No Violations Noted Drilling had been completed 07/13/10. Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Well-site had been properly restored and the discussed additional erosion controls from inspections in March and April 2011 had been implemented. No accelerated erosion or sedimentation was observed in relation to the access road or well-site
2099463 Primary Facility 2012-09-12 Complaint Inspection No Violations Noted Well appeared to be equipped for production at the time of the inspection. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation in relation to the access road or well-site.
