1869105 Primary Facility 2010-03-10 Routine/Complete Inspection No Violations Noted Drilling had been completed at the time of the inspection. Well was being equipped for production. Observed no accelerated erosion & sedimentation with respect to the well-site. Noted that the access road erosion controls had not been implmented properly. Specifically the road-side ditches did not have any temporary erosion controls, and the culvert inlets and outlets did have effective temporary erosion controls to minimize accelerated erosion and sedimentation. The culvert outlets had hay bales placed at the outlets, however hay bales when utilized in this way do not provide effective sediment removal. When culverts are utilized in this manner a small rock lined sediment trap placed at the outlet is the most effective BMP for sediment removal.

Noted that there did not appear to be any accelerated erosion or sediment being transported offsite at the time of the inspection. However the well pad itself had been restored properly, specifically the pad had not been vegetated to meet the 70% uniform coverage or with clean durable rock capable of resisting accelerated erosion or sedimentation. Also the ditches which flow into the culverts had not been clean out and subsequently stabilized with vegetation or rock.

Brine tank located onsite and it appeared to be in sound condition. The site had been rgraded, seeded, and mulched. The operator should contine monitor the sit e to insure that the vegtative cover begins to grow and attains the 70% uniform coverage criteria. Noted that the collection ditch progressing from the access road across the well-site had not been properly stabilized with either clean durable rock or erosion control matting along with seeding. As constructed the collection ditch will not properly minimize accelerated erosion or sedimentation.
2030346 Primary Facility 2011-12-30 Compliance Evaluation No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Site had been regraded, seeded, mulched and vegetation had begun to take hold. There was no evidence of accelerated erosion or sedimentation in relation to the access road or well-site. The operator should continue to monitor the ruinoff collection ditch originating on the access road, traversing the well pad and conveying runoff off the of the restored well pad. The Department recommends that the operator permantly stabilize this ditch with clean durable rock.
