1803628 Primary Facility 2009-06-09 Routine/Complete Inspection No Violations Noted Drilling was ongoing at the time of the inspection. Below grade pit was located onsite and had more than the two feet of required freeboard. Filter fabric fence BMP appeared to have been installed properly. No accelerated erosion & sedimentation was observed on the well-site or related to the access road. Noted that the access road was a dirt road with no implemented BMP¿s to minimize accelerated erosion & sedimentation. The Department recommends that the operator implement BMP¿s (rock) to minimize the amount of sediment generated in relation to the access road and carried out onto SR 4006. Met onsite with operator representative Randy Morris who stated that the rig equipment will be moved off tomorrow and that plow and broom equipment will be onsite and that prior to stimulating the well the road will be heavily rocked due to the large amount of truck traffic that will accompany the fracing process.

The site had been restored and it was noted that it appeared a portion of the site had been used as a log landing during a logging operation. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that the access road required routine maintenance to insure its continued passability.

The well pad is restored and permanently stabilized. The well is properly labeled and equipped for production. A well tag is present at the well head. A site ID sign is posted at the entrance to the well pad. There are no violations noted for this inspection.
