Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1774281 Primary Facility 2009-03-06 Compliance Evaluation No Violations Noted No accelerated erosion & sedimentation was noted during the site inspection. Observed that the operator had implemented additional erosion control measures as outlined in the operator response sent on 02/12/09. These additional measures should effectively minimize accelerated erosion & sedimentation provided they are properly maintained.
1973885 Primary Facility 2011-05-17 Routine/Complete Inspection No Violations Noted Well is equipped for production. Tag located. There is no evidence of erosion or sedimentation on the well site or access road.
1984167 Primary Facility 2011-06-16 Routine/Complete Inspection No Violations Noted Operator submitted Application for Drilling or Altering a Well (Deepen a well- Proposed 5000' TD) dated 05-16-11.
2208319 Primary Facility 2013-09-17 Complaint Inspection No Violations Noted Well shut in on 7” x 2” swage. 15 psig on wellhead at time of inspection. At surface, zero percent (0%) of gas measured (MSA 62S combustible gas indicator) outside 9 5/8” conductor casing and within 9 5/8” x 7” annulus. Pit lined and freeboard maintained. Inspection conducted w/WQS G. Higgins.
2226686 Primary Facility 2013-12-05 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed. Site and access road controls had been implemented properly and there was no observed evidence of accelerated erosion or sedimentation in relation to the site or access road. Drill pit located onsite and it contained fluids and drill cuttings. The drill pit had more than the required two feet of lined freeboard.
2266943 Primary Facility 2014-04-30 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well was not equipped for production. Drill pit located onsite and it contained drill cuttings and fluids. Drill pit had more than two feet of lined freeboard. There was no observed accelerated erosion or sedimentation in relation to the site or access road. It was noted that the acess road leaving the site required routine maintenance to prevent any erosion issues from developing.
2279139 Primary Facility 2014-06-17 Routine/Complete Inspection No Violations Noted Drilling appeared to have been completed, well was not equipped for production. Drill pit located onsite and it contained drill cuttings and fluids. Drill pit had more than two feet of lined freeboard. There was no observed accelerated erosion or sedimentation in relation to the site or access road. Met onsite with operator representative Tyler Krukar to review the proposed site restoration extension and existing erosion controls.
2307054 Primary Facility 2014-09-25 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. Site had been regraded, seeded, mulched and the vegetation met 70% uniform coverage. There was no observed accelerated erosion or sedimenation in relation to the site or access road.
2776725 Primary Facility 2018-09-19 Routine/Complete Inspection No Violations Noted This well is tagged and in production. The annulus is open. The well head casing, piping, equipment, pump jack and grounded 100 bbl fiberglass fluid collection tank look ok.

Note: I checked the open annulus and the immediate area around this well for gas leaks utilizing a calibrated Altair 5X Meter and all readings came back at non-detect for the presence of any gas.
