1814259 Primary Facility 2009-07-21 Routine/Complete Inspection No Violations Noted Site constructed, drilling appeared to have been completed at the time of the inspection. Operator was filling the onsite pit to prepare for well stimulation. Below grade pit had more than the two feet of required freeboard at the time of the inspection. No accelerated erosion & sedimentation was observed in relation to the well-site or access road and all erosion control BMP's appeared to be installed and functioning properly. Operator should continue to monitor the site for accelerated erosion & sedimentation until final well development and restoration activities are completed.
1821905 Primary Facility 2009-08-06 Routine/Complete Inspection No Violations Noted drilling complete, freeboard ok, not equipped for production.
1902373 Primary Facility 2010-08-02 Routine/Complete Inspection No Violations Noted Drilling was completed on 07/16/09. Well appeared to be equipped for & in production at the time of the inspection. Brine tank located onsite and appeared to be in sound condition. Well-site is located in a farm field. The field was not cultivated this growing season. Observed no well tag located on any of the well apparatus. At this time the Department is requesting that the operator contact WQS Higgins via phone or correspondence and notify him no later than August 23, 20010 that the well has been properly tagged.
2213385 Primary Facility 2013-10-17 Routine/Complete Inspection No Violations Noted Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. There was no observed accelerated erosion or sedimentation related to the site or access road. It was noted that the well tag was no longer legible. The operator should have a new well tag placed on the well apparatus as soon as possible.
2462722 Primary Facility 2016-03-07 Routine/Complete Inspection No Violations Noted Well tagged-in production. Annulus open. Well head casing looks ok.
