Noted accelerated erosion & sedimentation had occurred and sediment had moved off-site. In response to the operator had implemented a second line of filter fabric fencing, however it was installed improperly, i.e. it had not been trenched in with only the stakes for the fencing pounded into the ground. In addition the access road had been rocked, however the drainage ditch and subsequent turnout did not have any erosion control measures implemented to minimize erosion & sedimentation.
