Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1747492 Primary Facility 2008-10-16 Routine/Complete Inspection No Violations Noted Well site was not constructed. Noted the following deficiencies in the submitted E&S plan: 1) No BMP's shown as to be implemented on the access road. 2) Noted that there were numerous other swales the access road would cross separate from the swale noted as being 30 feet from the well pad. 3) The piled brush should be moved to the side of the well-site to keep the disturbed area further away from Truby Hill Rd. and the unamed tributary to Guffy Run. 4) The BMP located between the site and Truby Hill Rd. should be fitler fabric fence backed with haybales for stability. 5) A diversion ditch should be installed above the well-site so as to divert surface runoff from flowing towards the site.
1794617 Primary Facility 2009-04-30 Routine/Complete Inspection No Violations Noted Well is in post drilling pre-restoration phase, but not yet in production at the time of the inspection. The filter fabric fence is preventing any accelerated erosion and sedimentation. The erosion and sedimentation controls should be maintained (filter fabric fence) to avoid to much deposition of sediment against the fence. The operator should also monitor the access road and well site for erosion sedimentation until final restoration activities are complete. Pit is properly installed and within freeboard requirements. Accompanied Greg Higgins for training.
