1955479 Primary Facility 2011-03-04 Routine/Complete Inspection No Violations Noted Drilling was completed on 10/26/08. Well appeared to be equipped for production. Brine tank located onsite and it appeared to be in sound condition. No accelerated erosion was observed in relation to the site. Noted that the well was located in an existing pasture field. Excessive runoff flow had collected and washed over the access road making it impassable. Spoke with operator representative Mike Gulish and Mr. Gulish stated that the washed out portion of the access road was already on a list of wells that required work and that measures will be taken to insure that the same problem does not occur again.
