2554346 Primary Facility 2017-01-18 Incident- Response to Accident or Event Violation(s) Noted Inspection performed in response to Operator’s reported release of 29bbl (1218gal). production water to the ground on 1/17/2017. According to the Operator, the spill occurred due to a crack at the base of the tank caused by an animal burrowing beneath the tank. Operator estimated the release volume based on weekly tank readings that were previously obtained.

At the time of inspection, Operator on site, clean-up work ongoing. During inspection, Department confirmed Operator's reported cause of spill as indicated by a visible hairline crack at the base of the brine tank. Compromised brine tank disconnected from production equipment and moved to enable access to impacted area. Impacted area measuring approximately 8’x8’ being excavated and material placed in two roll off containers for proper disposal. Excavation of impacted area will be performed until all impacted material is recovered as indicated by conductivity measurements. Site appears to be stable with no E&S issues.

2559899 Primary Facility 2017-02-08 Compliance Evaluation No Violations Noted Inspection performed as a follow-up after receipt of the documentation requested in the 01/18/17 inspection. Excavation of the affected media had been completed and said media was contained in two covered roll-off boxes located onsite. Upon completion of confirmatory sampling and analysis the site will be backfilled and stabilized. The operator has also submitted documentation the operator will enter the Act 2 voluntary clean-up process.
