2685331 Primary Facility 2018-01-17 Compliance Evaluation Outstanding Violations - Viols Req'd This well is tagged, in production and in ongoing violation status as two large trucks, one load of production tubing, one drilling rig, one rig doghouse, one large ditching machine and various pumps and tanks have been left sitting on this property against the wishes of the landowner for over 5 years now. The property owner called the PA State Police to this site over 3 years ago to try and get all of this unwanted equipment removed but they were not able to help her and informed her that this is a DEP Oil & Gas matter. I have issued a previous NOV to the operator for this ongoing violation but the operator has ignored this violation. In addition, the operator has failed to submit either the required annual well mechanical integrity report or the required annual well production report.
2855278 Primary Facility 2019-03-19 Compliance Evaluation Outstanding Violations - Viols Req'd This is a compliance evaluation follow up inspection to check and see if the service rig and other equipment not necessary for the production of this well has been removed yet according to the order to do so.

NOTE: According to a letter received from the operator regarding this well dated January 27th, 2019, the operator stated that some equipment was removed from this well site and the rig was laid down and ready to move. Some equipment was removed from this site nearly a year ago but much still remains and as the photo attached to this report shows, the service rig was not laid down and ready to move.
